Özet

Sinonazal schwannom, sinir kılıfı hücrelerinden gelişen nadir ve genellikle yavaş büyüyen benign tümördür; nazal kavite ve etmoid sinüs sık yerleşimler arasındadır. Kontrastsız BT kitlenin sınırını ve basıya bağlı kemik genişlemesini gösterir; MR sinir ilişkisi, iç yapı ve kafa tabanı/kraniyal uzanım için daha bilgilendiricidir. Düzgün bası remodelingi agresif infiltrasyondan çok yavaş büyümeyi destekler, ancak tek başına kesin tanı koydurmaz. Kafa tabanına uzanımın gözden kaçması lezyonun sinir boyunca yaygınlığını eksik bırakabilir.

Faz ve pencere

Kontrastsız tanısal
Kontrastsız BT'de iyi sınırlı yumuşak doku dansitesinde kitle izlenir; kemik penceresinde komşu etmoid veya sinüs yapılarında düzgün genişleme ve basıya bağlı erozyon görülebilir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • Nazal kavite, etmoid veya sfenoid sinüste iyi sınırlı yumuşak doku kitlesi.
  • Komşu sinüs duvarlarında düzgün genişleme ve bası incelmesi.
  • Kortikal sınırda keskin bası erozyonu; düzensiz permeatif yıkımın bulunmaması.
  • Kitleyle birlikte nazal kavite ve birden çok sinüsün basıyla genişlemesi.
  • Kontrastlı BT'de hafif veya orta derecede boyanma izlenebilir; MR görüntülerde ise belirgin boyanma beklenir ve juvenil anjiofibroma gibi aşırı hipervasküler paternler tipik değildir.
  • Kistik veya hemorajik odaklara bağlı iç yapı heterojenliği.
  • Kafa tabanı ya da kraniyal sinir foramenlerine doğru uzanım kuşkusu.

Normalde

Normal etmoid hücreler hava dolu ve ince kemik septalarla ayrılmıştır; nazal kavitede konka ve septum konturları simetriktir. Karşı etmoid hücrelerle septa aralığını kıyasla ve kitlenin korteksi düzgün dışa itip ittiğini, yoksa düzensiz biçimde ortadan kaldırdığını izle.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Antrokoanal polip
maksiller antrumdan ostium ve koanaya uzanan sıvı/mukoza yoğunluklu yapı.
Juvenil anjiofibrom
posterior nazal merkezli ve belirgin kontrastlanan hipervasküler tümör.
Sinonazal nörofibrom
sinir kılıfı kaynaklı olabilir; görüntüleme bulguları schwannomla örtüşür ve histoloji gerekir.

Tuzaklar

  • Yavaş büyüyen benign schwannomda bası remodelingini malign invazyon diye adlandırma; kortikal kenarın düzgünlüğünü takip et.
  • Basit kist ya da retansiyon lezyonu da sinüsü genişletebilir; solid yumuşak doku komponentini ayır.
  • BT kafa tabanı kemik kanalını gösterir, sinir boyunca yumuşak doku uzanımını tam dışlamaz; MR gereksinimini not et.
