Özet

Tünelli hemodiyaliz kateterleri, tekrarlayan ekstrakorporeal erişim gerektiren hastalarda santral venöz sistem ve sağ atriyuma uzanır. Uç malpozisyonu, kateter çevresi fibrin kılıfı/trombüs ve santral ven darlığı kateter işlevini bozabilir ve gelecekteki damar erişim yollarını etkileyebilir. BT, kateter seyrini, santral venöz açıklığı, gelişen kollateralleri ve olası mediastinal komplikasyonları tek incelemede değerlendirebilir; kateter disfonksiyonunun nedenleri olan stenoz, tromboz ve fibrin kılıfı bu yöntemle tespit edilebilir. Santral venöz oklüzyonlar, düzgün röntgen ve ultrason bulgularıyla seyrederek kateter disfonksiyonu veya erişim kaybı belirtileri ortaya çıkana kadar saptanmayabilir.

Faz ve pencere

Kontrastsız tanısal
Kateter pozisyonu ve işlev bozukluğunun değerlendirilmesinde göğüs grafisi yetersiz kalabilmekte olup, kateter yerleşimi ile santral venöz stenoz, tromboz ve fibrin kılıf oluşumunu tespit etmek amacıyla kontrastlı çok detektörlü BT venografisi (MDCTV) uygulanır.
Venöz tanısal
Kontrastlı BT venografisi ile kateter boyunca duvara yapışık dolum defektleri, vena kava superior veya brakiosefalik ven stenozu/oklüzyonu, alternatif kollateral yollar ve ekstravazasyon gibi komplikasyonlar değerlendirilebilir.

Önerilen pencereler: Mediasten (G 350 / M 50), Anjiyo (G 600 / M 150), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Kateterin brakiosefalik ven ve vena kava superior boyunca sağ atriyuma ilerlememesi, damar duvarına yaslanması ya da sağ ventriküle uzanması.
  • Kateter ucunu çevreleyen veya yan deliklere doğru uzanan ince kontrastlı kılıf ile katetere bitişik kalıcı dolum defekti; fibrin kılıfı/trombüs olasılığı.
  • Vena kava superior veya brakiosefalik vende segmental daralma ya da tam opaklaşma kaybı; darlık öncesi ven genişlemesi eşlik edebilir.
  • Azygos, hemiazygos, internal torasik ve göğüs duvarı venlerinde belirginleşme santral çıkış engeline eşlik eden kollateral dolaşımı gösterir.
  • Kateter boyunca keskin kink, lümen süreksizliği, bağlantı ayrılması veya damar dışına uzanan segment mekanik komplikasyon lehinedir.
  • Kateter çevresindeki mediastinal hematom/sıvı veya kontrast kaçağı damar perforasyonunu düşündürür; plevral sıvı eşlik edebilir.

Normalde

Doğru pozisyonda kateter, cilt altı tünelinden giriş yaparak brakiosefalik ven ve vena kava superior boyunca ilerler ve distal ucu sağ atriyuma ulaşır. Venöz fazda santral ven lümeni düzgün ve homojen opaklaşır; kollateral damarlar belirginleşmez ve kateter çevresinde kalıcı dolum defekti görülmez.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Fibrin kılıfı
kateteri dıştan saran düzgün, ince bir kontrast halkası veya uçta retrograd olarak uzanan kontrast boşluğu ile kendini gösterebilir.
Akut santral ven trombozu
lümen içinde kalıcı kontrast defekti ve kimi zaman ven genişlemesiyle görünür.
Kronik santral ven stenozu
düzgün veya düzensiz uzun segment daralma, tıkanma ve gelişmiş kollaterallerle seyreder.
Kateter kink'i
damar lümeni tıkalı olmasa bile, kateter hatlarında keskin bükülme veya katlanma izlenebilir.
İlk geçiş kontrast karışımı
özellikle üst ekstremite enjeksiyonlarından sonra damar içinde yamalı atenüasyon yapar ve gecikmiş görüntüde düzelebilir.

Tuzaklar

  • HD kateterinin işlevsel uç seviyesi cihaz türü ve giriş tarafına göre değişebilir; yalnız kalp odacığına göre değil, damar içi konum ve önceki görüntüyle birlikte yorumla.
  • İlk geçiş BT venografide bilateral kol enjeksiyonlarının karışımı SVC'de trombüs benzeri odak oluşturabilir; gecikmiş fazda kaybolan heterojenliği gerçek duvar trombüsü sayma.
  • Sıkı kollateral damar ağı küçük ya da atlanmış bir santral ven darlığını maskeleyebilir; kollateral akışın toplandığı santral segmenti geriye doğru izle.
  • Katetere paralel fibrin kılıfını yalnız cihaz konturuna bakarak tanımak güç olabilir; çevre venin opaklaşmasını ve ucun distalindeki kontrast desenini değerlendir.

  1. Tünelli HD kateteri çevresindeki SVC defekti yalnız ilk geçiş serisinde görülüyor; gecikmiş seride ven homojen. En olası neden?

    Kontrast karışım artefaktı. İlk geçişteki heterojen venöz karışım gecikmiş opaklaşmayla kaybolabilir. Gerçek trombüs kalıcı dolum defekti, stenoz kalıcı lümen daralması, kırık ise kateter çizgisinde kopukluk yapar.
