Özet

Pankreas metastazı, bilinen veya daha önce tedavi edilmiş bir primer tümörün pankreas parankiminde tek odak, çoklu nodül ya da daha yaygın infiltrasyon şeklinde ortaya çıkmasıdır. Kontrastlanma paterni kaynak tümöre göre değişebilir; berrak hücreli renal hücreli karsinom metastazları arteriyel fazda hızlı ve belirgin kontrastlanabilir. Çok fazlı BT odak sayısını, pankreas kanalıyla ilişkisini ve pankreas dışı metastazları birlikte göstererek primer pankreatik tümörden ayrımı destekler. Metastazı yeni primer gibi yorumlamak evrelemeyi ve hastalığın kökenine ilişkin klinik değerlendirmeyi yanlış yönlendirebilir.

Faz ve pencere

Kontrastsız
Pankreas metastazları görüntülemede primer pankreatik maligniteleri taklit edebileceğinden, yalnız görüntüleme bulgularıyla ayırt edilmeleri güç olabilir.
Arteriyel tanısal
Pankreatik parankimal metastazlar BT’de arteriyel fazda maksimal kontrastlanma gösterebilir; renal hücreli karsinom pankreas metastazı da belirgin kontrastlanan bir kitle olarak görülebilir.
Portal tanısal
Portal venöz seride hipovasküler metastazlar pankreas parankimine göre düşük atenüiteli görünür; hipervasküler renal hücreli karsinom metastazlarının pankreasla atenüite farkı azalabilir. Uzak metastaz açısından eşlik eden hastalık bulguları da değerlendirilir.

BT bulguları

  • Fokal pankreatik nodül — tek, düzgün sınırlı veya infiltratif odak ana bez parankiminden ayrılır.
  • Multifokal dağılım — aynı bezin farklı bölümlerinde birden çok ayrı odak metastaz olasılığını artırır.
  • Hipervasküler kontrastlanma — pankreatik parankimal metastazlarda arteriyel fazda maksimal kontrastlanma görülebilir; renal hücreli karsinom metastazı da BT'de belirgin kontrastlanabilir.
  • Hipovasküler odak — akciğer veya gastrointestinal kaynaklı metastaz pankreas dokusuna göre daha az kontrastlanabilir.
  • Melanom pankreas metastazı, BT’de düşük atenüasyonlu ve yüksek atenüasyonlu periferik rimli bir kitle olarak görülebilir; büyük lezyonlarda periferik rim kontrastlanması ve santral nekroz tanımlanmıştır.
  • Ana pankreatik kanal etkisi — kanal basısı veya tıkanması görülebilir; kanal genişlemesinin yokluğu metastazı dışlamaz.
  • Eşzamanlı uzak hastalık — karaciğer, akciğer, kemik, adrenal veya peritoneal metastazların dağılımı primer tümör öyküsüyle karşılaştırılır.
  • Pankreasta metastaz olasılığı, özellikle renal hücreli karsinom, akciğer kanseri veya melanom öyküsü bulunan hastalarda; klinik öykü ve pankreas odağının görüntüleme özellikleri birlikte değerlendirilerek düşünülmelidir.

Normalde

Pankreatik parankimal metastazlarda Wirsung kanal genişlemesi olmayabilir; bu nedenle genişlemenin yokluğu metastazı dışlamaz.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

BT'de düşük veya karışık yoğunluklu, damarlanması az bir kitle ve pankreatik kanal genişlemesi gösterebilir.
Pankreatik lenfoma
çoğunlukla büyük ve daha homojen kitle görünümündedir; damarları sarmasına karşın lümeni açık bırakabilir ve çevresel lenfadenopati eşlik edebilir.
İntrapankreatik aksesuar dalak
kuyrukta dalakla aynı fazlarda aynı kontrast davranışını gösterir ve seri kesitlerde dalak dokusuna benzer.
Pankreasın inflamatuvar kitleleri maligniteyi taklit edebilir; takip BT'sinde küçülme inflamatuvar lezyon olasılığını destekleyebilir.

Tuzaklar

  • Arteriyel fazda maksimal kontrastlanma gösteren lezyonlar pankreatik parenşimal metastazı destekler; yalnız portal fazda değerlendirilirse kaçırmaya yol açabilir.
  • Hipervasküler pankreatik odak doğrudan nöroendokrin primer anlamına gelmez; bilinen renal hücreli karsinomda pankreas metastazı güçlü bir alternatiftir.
  • Tek metastazın duktal obstrüksiyon yapabileceğini unutmayın; kanal genişlemesi yokluğu metastaz tanısını dışlayan bir kriter değildir.
  • Küçük lezyonları yalnız kontrastsız seride aramayın; nodülün çevre parankime göre farkı kontrast fazına bağlı olarak değişebilir.
