Özet

Talasemi ve diğer kronik hemolitik anemilerde artmış eritropoez dalakta büyümeye, kemik iliğinde genişlemeye ve bazen kemik dışı hematopoetik kitlelere yol açar; görünüm hastalık yükü ve transfüzyon öyküsüne göre değişir. Portal venöz kontrastlı abdomen BT dalak hacmini ve eşlik eden karaciğer/parankim bulgularını gösterirken kemik algoritması iliğin genişlemesini, kortikal incelmeyi ve trabeküler yeniden yapılanmayı ortaya koyar. Posterior mediastinal veya paraspinal kitlelerin EMH olabileceğini fark etmek gereksiz biyopsi riskini azaltır; spinal kanala uzanımın gözden kaçması nörolojik basıyı atlatabilir. Dalak büyümesini ya da kemik değişikliğini tek başına belirli bir hemoglobinopatiye özgü saymak doğru değildir.

Portal tanısal
Abdominal görüntüleme ile dalak boyutu, parankim yapısı ve eşlik eden karaciğer bulguları birlikte değerlendirilir. Dalak boyutu ve parankim yapısı standart abdominal protokol ile değerlendirilir.
Kemik algoritması tanısal
Aksiyel iskelet ve kaburgalarda medüller genişleme, kortikal incelme, trabeküler belirginleşme ve paraosseöz yumuşak doku komponenti kemik rekonstrüksiyonlarında incelenir; kemik algoritması kontrast fazı değildir.

Önerilen pencereler: Batın (G 400 / M 50), Kemik (G 1800 / M 400), Mediasten (G 350 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Splenomegali — dalak büyümüş olabilir; boyut hastalık şiddeti, tedavi ve eşlik eden portal hipertansiyondan etkilenir.
  • Dalak parankiminde heterojenlik, nodüler ekstramedüller hematopoez veya vasküler lezyonlar nedeniyle görülebilir; bu bulgu özgül değildir.
  • Medüller kavite genişlemesi — vertebra, kaburga, pelvis veya uzun kemiklerde hematopoetik iliğin genişlemesine bağlı trabeküler görünüm değişir.
  • Kortikal incelme ve osteopeni — genişlemiş medüller alan korteksi inceltip kemik yoğunluğunu azaltabilir.
  • Kaburga şekil değişikliği — posterior kaburgalarda genişleme ve iç kortekste düzensizlik paraosseöz hematopoezle ilişkili olabilir.
  • Paraspinal veya posterior mediastende yerleşimli, iyi sınırlı yumuşak doku kitlesi şeklinde EMH, spinal kanala uzanarak kompresyon yapabilir.
  • Ekstramedüller yumuşak doku — toraks, retroperiton veya presakral alandaki odaklar kemik iliği genişlemesi ve hematolojik öyküyle birlikte yorumlanır.

Normalde

Komşu vertebra-kaburga düzeylerini ve önceki incelemeleri karşılaştırırken korteks kalınlığı, medüller trabekülasyon ve paraspinal yumuşak doku kitlesinin kemikle ilişkisini birlikte değerlendir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
