Özet

Granülomatöz sialadenit, tüberküloz ve atipik mikobakterilerden sarkoidoz, aktinomikoz ve diğer sistemik hastalıklara uzanan farklı nedenlerin tükürük bezinde oluşturduğu histolojik bir örüntüdür. Kontrastlı BT, bez büyümesini, çevre dokudaki inflamasyonu, nodal hastalığı ve apse gibi komplikasyonları haritalar; tek başına hangi etkenin sorumlu olduğunu belirleyemez. Sarkozidoz bağlamında tükürük bezi tutulumu radyolojik olarak tümörleri taklit edebilir; mikobakteriyel enfeksiyonlarda ise bez içinde apse ve lenf nodlarında nekrotik değişiklikler görülebilir. İnflamasyonun fasyal boşluklara uzanımı ve sinüs/fistül oluşumu hastalığın yayılımını gösterir; kesin tanı için mikrobiyolojik ve histopatolojik incelemeler gereklidir.

Faz ve pencere

Kontrastsız
Kontrastsız BT'de tükürük taşı ve kalsifikasyonlar belirgin olabilir; ancak bez içi yumuşak doku inflamasyonu ile apse duvarı arasında güvenilir ayrım yapılamaz.
Kontrastlı tanısal
Kontrastlı boyun BT'sinde bez büyümesi ve heterojen parankim kontrastlanması, odaksal sıvı koleksiyonu varsa çevresel kontrastlanan duvar, ayrıca nekrotik intraparotid/periparotid veya servikal nodlar aranır. Mikobakteriyel enfeksiyonlarda bez içinde apse ve lenf nodlarında nekrotik değişiklikler görülebilir; kesin etken tanısı için mikrobiyolojik doğrulama şarttır.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • Parotis veya submandibular bezde diffüz büyüme ya da fokal, tümörü taklit eden yumuşak doku odağı
  • Kontrastlı incelemede bez parankiminin heterojen kontrastlanması ve çevre yağ dokusunda inflamatuvar kirlenme
  • Tüberküloz veya atipik mikobakteri tutulumu ile uyumlu olabilen santral nekrozlu intraparotid ya da servikal nodlar
  • Bez içinde veya hastalıklı nodlarda kalsifikasyon; bu bulgu özellikle kronik enfeksiyöz süreçte görülebilir
  • Bez içinde küçük, çevresel kontrastlanan sıvı odağı veya birleşerek apse oluşturan koleksiyon
  • Aktinomikozda düzensiz yumuşak doku inflamasyonunun deriye uzanması ve sinüs/fistül yolları görülebilir.
  • Sarkoidozda tek ya da iki taraflı parotis büyümesi ve intraparotid yumuşak doku nodülleri; BT görünümü özgül değildir
  • Submandibular bez, parotis ve komşu nodların aynı ya da farklı taraflarda birlikte tutulabilmesi

Normalde

Tükürük bezleri yüzeyel yapılar olup ultrasonografi ile kolayca değerlendirilebilir; boyundaki fasyal boşluklar ve kompartmanlar patoloji yayılımının anlaşılması için önemlidir. Sağ ve sol tükürük bezlerinin boyutları ve parankim özellikleri karşılaştırılır; nodüler lezyonlarda hilüs yapısının korunumu veya santral nekroz değerlendirilir, inflamatuar sürecin komşu fasyal boşluklara ve cilt altı dokuya yayılımı izlenir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Akut bakteriyel sialadenit
bez büyümesi ve çevresel yağ dokusu ödemi yapar; obstrüksiyon/taş veya sınırlı apse görülebilir, granülomatöz etkeni BT ile dışlamaz.
Tükürük bezi lenfoması
bez içinde homojen yumuşak doku kitlesi ve nodal hastalık oluşturabilir; bez mimarisini saran diffüz büyüme olabilir, tanı doku örneğine dayanır.
Metastatik intraparotid lenf nodu
özellikle baş-boyun deri tümörü öyküsü ve santral nod nekrozu metastazı destekler; parotis parankiminden ayrımı nodal şekil ve dağılımla yapılır.
Sjögren sendromunda tükürük bezlerinde otoimmün inflamatuar süreç gözlenir; lenfoma riski nedeniyle lenfoproliferatif kitleler ayrıca değerlendirilmelidir.
IgG4 ilişkili sialadenit
çoğu kez iki taraflı ve birden çok bezde büyüme yapar; diğer organ bulguları ile histoloji ayırıcıdır.

Tuzaklar

  • Nekrotik servikal nodu doğrudan metastaz kabul etme; tüberküloz ve atipik mikobakteriler de santral nekroz, çevresel kontrastlanma ve bazen kalsifikasyon yapabilir.
  • Fokal parotis odağını otomatik olarak primer tükürük bezi tümörü sayma; intraparotid lenf nodu, granülomatöz odak veya apse benzer yumuşak doku yoğunluğu oluşturabilir.
  • Sarkoidoz, mikobakteriyel enfeksiyon ve diğer granülomatöz nedenleri BT görünümüyle kesin ayırma; klinik, laboratuvar ve gerektiğinde doku incelemesi şarttır.
  • Koleksiyon erken dönemde belirginleşmeyebilir; çevre yağ dokusu inflamasyonu, cilt fistülü ve komşu boşluklara uzanım da enfeksiyon yayılım haritasına dahildir.
