Özet

Kimura hastalığı, çoğunlukla genç erişkin erkeklerde görülen, baş-boyun deri altı dokusu, majör tükürük bezleri ve bölgesel lenf nodlarını tutabilen kronik inflamatuvar bir hastalıktır. Kontrastlı BT, parotis içi veya çevresindeki odakların dağılımını, infiltratif uzanımı ve eşlik eden servikal lenfadenopatiyi birlikte göstererek kitle değerlendirmesine katkı sağlar; görünüm tek başına özgül değildir. Deri altı yumuşak doku kitleleri, homojen hiperatenuasyonlu lenf nodları ve tükürük bezi büyümesi birlikte Kimura hastalığını düşündürebilir; klinik öykü, eozinofili ve yüksek IgE tanıya yardımcı olur, kesin tanı için sıklıkla doku örneklemesi gerekir. Bu tabloyu yalnızca parotis tümörü olarak ele almak eşlik eden deri altı kitlelerin ve lenf nodlarının dağılımını gözden kaçırabilir; görüntüleme bulguları özgül olmadığından maligniteyi dışlamak için histolojik inceleme gerekir.

Kontrastlı tanısal
Kontrastlı BT'de parotis ve deri altı dokularda sınırları belirsiz yumuşak doku genişlemeleri ve lenf nodlarında homojen hiperattenüasyon görülebilir; ileri olgularda heterojen kontrastlanma da izlenebilir. Servikal ve intraparotid nodlar çoğunlukla homojen kontrastlanır; bu kesitsel görünüm histolojik tanı koydurmaz.

Normalde

Kimura hastalığında kontrastlı BT'de sınırları belirsiz büyümüş deri altı kitleler, homojen hiperattenüe lenf nodları ve şişmiş tükürük bezleri birlikte görülebilir. Kimura hastalığında BT, deri altı kitleleri ve eşlik eden bölgesel lenfadenopatiyi göstermeye yardımcı olabilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Lenfoma
Kimura hastalığı görüntülemede lenfoma gibi neoplastik hastalıkları taklit edebilir; görüntüleme bulguları özgül olmadığından histolojik inceleme tanıda belirleyicidir.
Metastatik servikal lenfadenopati
mukozal veya akciğer kökenli maligniteler lenf nodlarında tutulum yaparak Kimura hastalığı ile karışabilir; primer kitle aranmalıdır.
Tüberküloz lenfadenitinde kontrastlı US incelemede nodal hilusun kaybı, nekroz, heterojen kontrastlanma ve periferik halka tarzı kontrastlanma ayırıcı tanıda değerlendirilebilir.
Pleomorfik adenom
parotis tutulumu gösteren Kimura hastalığı parotis tümörü izlenimi verebilir; görüntüleme bulguları tek başına kesin ayrım sağlamaz.
IgG4 ilişkili hastalık, Kimura hastalığının ayırıcı tanısında değerlendirilebilir; bir olgu raporunda inceleme sonrası dışlanmıştır.
Anjiyolenfoid hiperplazi ve eozinofili (ALHE), baş-boyunda deri nodülleri yaparak Kimura hastalığını taklit edebilir; Kimura hastalığında derin subkutan ve tükürük bezi tutulumu ile bölgesel lenfadenopati daha belirgin olabilir.

Tuzaklar

  • Homojen kontrastlanan servikal nodları yalnız boyutları nedeniyle malign sayma; Kimura hastalığında reaktif nodlar belirgin büyüyebilir ve BT morfolojisi özgül değildir.
