Özet

İntraparotid lenf nodları özellikle kulak, saçlı deri ve yüz derisindeki kutanöz skuamöz hücreli kanser ile melanomun bölgesel yayılım yolu olabilir. Kontrastlı boyun BT, parotis içindeki şüpheli nodu, çevre yumuşak dokuya yayılımı ve eşlik eden boyun nodlarını aynı incelemede haritalar. Yuvarlaklaşma, düzensiz sınır ve santral nekroz kuşku uyandırır; ancak görüntü tek başına metastazı primer parotis tümöründen kesin ayıramaz. Cilt kanseri öyküsü gözden kaçarsa nodal hastalık yanlışlıkla bezin primer tümörü sayılabilir ve bölgesel yayılım eksik raporlanabilir.

Faz ve pencere

Kontrastlı tanısal
Kontrastlı BT'de parotis içindeki nodüler odak heterojen kistik-solüt yapıda veya enfiltratif yumuşak doku olarak görünürken, eşlik eden servikal nodlarda santral nekroz veya düzensiz sınır izlenebilir; parotis ve servikal bölge aynı incelemeye dahil edilerek bölgesel tutulum haritalanır.
Kontrastsız
Nodun yerleşimi ve boyutu seçilebilir; nekroz, çevresel kontrastlanma ve parotis dışına uzanımın ayırımı sınırlı olduğundan tek başına tanısal değildir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40).

BT bulguları

  • Parotis bezinin yüzeyel veya derin lobunda, primer cilt lezyonunun drenaj alanına uygun nodüler kitle veya büyüklüğü artmış lenf nodu izlenebilir.
  • Servikal lenf nodlarında görülebilen santral nekroz, baş ve boyun skuamöz hücreli kanser metastazında beklenen bir bulgudur.
  • Düzensiz ya da belirsiz nod sınırı ve çevre yağ planlarında silinme, ekstranodal yayılım kuşkusu doğurur
  • Metastatik tutulum sıklıkla parotis içi lenf nodlarının yanı sıra alt çene altı ve servikal lenf düğümlerinde de eşlik eder.
  • İpsilateral üst juguler, özellikle II. düzey nodların birlikte tutulumu
  • Kulak kepçesi, dış kulak yolu, şakak, saçlı deri veya yüz derisindeki bilinen/şüpheli primer cilt lezyonu ile aynı taraflı nodal hastalık
  • Parotis içi metastatik lezyonların radyolojik özellikleri primer parotis tümörleriyle örtüşebildiğinden, ayırıcı tanı için klinik öykü kritiktir.

Normalde

Normal parotis içinde küçük intraglandüler nodlar görülebilir; oval biçimleri ve çevre bez dokusuyla uyumlu düzgün konturları metastazı tek başına kanıtlamaz. Tek taraflı bir lezyonda bile, kontralateral parotis bezinin radyolojik değerlendirmesi eşlik eden bilateral tutulumu ekarte etmek amacıyla yapılmalıdır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Warthin tümörü, parotide en sık görülen benign neoplazmalardan olup, genellikle kistik ve solid komponentleri olan, kontrast alan iyi sınırlı bir kitledir.
Lenfoepitelyal karsinom, parotide tek veya çok odaklı kitleler şeklinde ortaya çıkabilir; primer bez tümörlerinden klinik ve radyolojik olarak ayrılmalıdır.
Reaktif veya inflamatuar lenfadenopatiler (örneğin Kikuchi-Fujimoto hastalığı gibi nekrotizan lenfadinitler), servisalde sık görülür ve maligniteyle karışabilir. Kikuchi-Fujimoto hastalığı), servikal zincirde sık görülen, benign ve oto-limite edici bir nekrotizan lenfadinittir ve lenfoma ile karışabilir. Metastatik parotis kitleleri genellikle ağrısız ve yavaş büyüyen bir kitle olarak bulunur; doğru tanı için ipsilateral cilt primerinin öyküsü kritik önem taşır.

Tuzaklar
