Tiroid metastazı BT bulguları

Özet

Tiroid metastazı nadir görülür ve çoğu zaman bilinen ya da geçirilmiş başka bir malignitesi bulunan hastada saptanır; böbrek hücreli karsinom, akciğer, meme ve diğer solid tümörler kaynak olabilir. Kontrastlı BT tiroid odaklarının damar yapıları ve servikal nodlarla ilişkisini gösterirken, eşlik eden sistemik hastalık toraks ve batın dahil geniş tarama alanlarında değerlendirilir. Görüntü görünümü primer tiroid tümörleriyle örtüştüğünden klinik öykü ve önceki incelemeler önemlidir. Geçmişteki malignite öyküsü sorgulanmazsa metastaz primer tiroid kanseri olarak yanlış yorumlanabilir ve yaygın hastalık kapsamı gözden kaçabilir.

Faz ve pencere

Venöz tanısal: Kontrastlı boyun yumuşak doku görüntülerinde intratiroidal bir veya birden çok solid odak, heterojen ya da yoğun kontrastlanma ve bez konturunda kabarıklık oluşturabilir; servikal nodlar ile karotis ve juguler ven ilişkisi incelenir.
Kontrastsız: Metastatik odaklar çevre tiroid parankiminden farklı yumuşak doku dansitesinde görünür; hızlı büyüme nedeniyle nekroz veya kistik dejenerasyon saptanabilir. Kontrastsız görünüm metastazı primer tiroid neoplazisinden ayırt ettirmez.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Akciğer (G 1500 / M -600), Batın (G 400 / M 50), Kemik (G 1800 / M 400).

BT bulguları

Metastatik odak tiroid lobunda parankimden ayrılan solid yumuşak doku nodülü olarak görünebilir.
Tiroid metastazlarında çoklu nodüller sık görülür; tiroidin yoğun vasküler yapısı nedeniyle metastatik depozitler genellikle hematojen yolla oluşur.
Heterojen ya da belirgin kontrastlanma — vasküler primerlerden, özellikle renal hücreli karsinomdan gelen odaklar belirgin kontrastlanabilir.
Tiroid dışı yayılım — odak kapsül dışına uzanıp trakea, özofagus veya boyun damarlarına komşuluk gösterebilir.
Santral veya lateral servikal nodal hastalık nadiren eşlik eder; bu nedenle koruyucu boyun diseksiyonu genellikle önerilmez.
Sistemik metastatik yük — aynı inceleme kapsamındaki akciğer, karaciğer, kemik veya lenf nodu odakları tiroid lezyonunun metastatik bağlamını destekler.
Önceki malignite öyküsüyle uyum — önceki görüntülerde yeni gelişen ya da büyüyen tiroid odağı tanısal bağlam sağlar.

Normalde

Normal tiroid parankimi iki lobda homojen ve simetrik konturlu görünür; belirgin solid odak ya da kapsül dışı yumuşak doku bulunmaz. Lezyonun yerini, sayısı ve komşu yapılarla ilişkisi değerlendirilirken; sistemik metastatik yük ve primer tümörün özellikleri multidisipliner yaklaşımla birlikte tartılır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Primer diferansiye tiroid karsinomu: intratiroidal nodül ve servikal nodal yayılım yapabilir; ultrason ve sitolojik incelemenin yetersizliği durumunda primer ile metastatik arasındaki ayrım için immünhistokimya ve moleküler markerlar gereklidir.
Medüller tiroid karsinomu: primer ve metastatik odaklarda kalsifikasyon görülebilir; klinik biyokimya ve doku tanısı gereklidir.
Anaplastik tiroid karsinomu: büyük, infiltratif, nekrotik kitle ve yaygın lokal invazyon daha tipiktir.
Primer tiroid lenfoması: diffüz bez büyümesi veya görece homojen fokal kitle, özellikle Hashimoto zemininde görülebilir.
Benign tiroid adenomu veya nodüler guatr: iyi sınırlı, kistik dejenerasyonlu ya da kalsifiye nodül olarak görünebilir; ancak metastatik hastalık öyküsü varlığında bu bulgular metastazı ekarte etmez.

Tuzaklar
