Özet

Retina dekolmanı cerrahisinde kullanılan silikon yağ ve çevresel bantlar travma BT'sinde kanamayla karışabilir; katarakt cerrahisi sonrası intraoküler lens BT'de yüksek atenüasyonlu görünür. BT, skleral çökertme materyallerinin ve vitreus içindeki gaz veya silikon yağ tamponadının görünümünü değerlendirmeye yardımcı olur; katarakt cerrahisi sonrası intraoküler lens yüksek atenüasyonlu izlenebilir. Bu materyalleri kanama sanmak gereksiz incelemeye yol açabilir; travmada BT'nin tüm açık glob yaralanmalarını saptayamayabileceği de göz önünde bulundurulmalıdır. İki globun dış konturunu ve cerrahi materyalin anatomik yerleşimini birlikte okuyarak yorumla.

Skleral çökertme materyalleri göz duvarında indentasyon oluşturabilir; vitreus içindeki gaz BT'de hava atenüasyonunda, silikon yağı ise hiperatenuasyonlu görünür. Enükleasyon sonrası orbital protez kullanılabilir.

BT bulguları

• Skleral çökertme bandı, glob duvarının dışından geçen ince çevresel kontur oluşturur ve komşu sklerada hafif içe çekilme yapabilir.
• Vitreus boşluğunu dolduran silikon yağı belirgin hiperdens olabilir; dansiteyi karşı göz ve görüntü gürültüsüyle kıyasla.
• Retina cerrahisi sonrası intravitreal gaz glob içinde keskin sınırlı, hava atenüasyonlu kabarcık veya hava-sıvı seviyesi biçimindedir.
• Katarakt cerrahisinde doğal lensin yerinde ince, yüksek atenüasyonlu intraoküler lens görülür.
• Enükleasyon sonrası orbital implant ve protez kullanılabilir.
• İmplantın beklenen küresel yatağından yer değiştirmesi, çevre yumuşak dokuda yeni şişlik veya kemik duvarında eşlik eden değişiklik komplikasyon şüphesi doğurur.
