Yüz yumuşak doku hematomu BT bulguları

Özet

Künt veya penetran yüz travmasında hematom yüzeyel yağ dokusunda, masseter-bukkal kompartımanda, ağız tabanında ya da orbit çevresinde gelişebilir. Çok kesitli BT, yüz travmasında kırıkları ve ilişkili komplikasyonları değerlendirmede tercih edilen yöntemdir; damar yaralanması şüphesinde BT anjiyografi damar lezyonlarını araştırmak için kullanılabilir. Hematomun anatomik boşluklar boyunca yayılımı ve dil/ağız tabanı üzerindeki kitle etkisi önemlidir. Yüz travmasında orbital kompartman sendromu ve dinamik hava yolu obstrüksiyonu gibi zaman duyarlı aciller erken tanınmalıdır.

Okuma sırası

Yüz ve ağız tabanı yumuşak dokuları. Yüz travması BT'sinde kırıkları saptayıp ilişkili yumuşak doku yaralanmalarını da inceleyin.
Hematoma komşu yumuşak doku. Ardışık BT kesitlerinde damar dışında görülen kontrast odağının progresif artışını inceleyin.
Dil ve hava kolonu. Yüz ve boyun travmalarında hava yolu değerlendirmesine öncelik verin; dinamik obstrüksiyon riski göz önünde bulundurulmalıdır.
Orofaringeal hava yolu. Retrofaringeal boşluğun kafa tabanından üst mediastene uzanımını ve bu boşluktaki lezyonların aerod sindirim yolu üzerindeki olası etkilerini değerlendirin.
Komşu damarlar ve yüz kemikleri. BT'de yüz kemik kırıklarını değerlendirin; penetran boyun yaralanmasında damar yaralanması şüphesi varsa BT anjiyografi kullanın.

Faz ve pencere

Kontrastsız tanısal: Orbital subperiosteal hematom orbitin üst kesiminde daha sık görülse de yerleşimi orbit içinde değişkenlik gösterebilir.
BTA tanısal: BT anjiyografide aktif kanama şüphesiyle damar dışında kontrast göllenmesi ve damar duvar düzensizliği aranır; pseudoanevrizma ve damar oklüzyonu gibi bulgular ayrıca değerlendirilir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400), Anjiyo (G 600 / M 150).

BT bulguları

Orbital subperiosteal hematom travmayla ilişkili olabilir; orbit içindeki yerleşimi değişkenlik gösterebilir.
Yüz travmasında yumuşak doku hasarı, lokal şişlik ve komşu yağ planlarında kirlenme olarak izlenebilir.
Travma kaynaklı koleksiyonlar, fasyal boşluklar boyunca (buccal, parotis, parafaringeal) yayılım gösterebilir.
Ağız tabanı kitle etkisi — sublingual kanamanın dili yukarı ve arkaya itmesi ya da orofaringeal hava kolonunu daraltması.
BT anjiyografide aktif kanama belirtisi olarak damar dışı fokal kontrast göllenmesi (spot sign) ve damar dışında ilerleyen kontrast izlenir.
Travmaya bağlı vasküler yaralanma bulguları arasında damar içi intimal flap, damar duvar düzensizliği, lümen daralması ve pseudoanevrizma yer alır.
Yüz travmaları genellikle kırıklarla birlikte görülür; penetran travmalarda yara hattı boyunca yabancı cisim veya kemik fragmanları aranmalıdır.
Orbital bölgede subperiosteal veya retrobulber kanama gelişebilir; bu durum kütle etkisine yol açarak orbital yapıları geriletebilir.
Orbital subperiosteal hematomun orbit içindeki yerleşimi değişken olabilir ve travmayla ilişkili olabilir.

Normalde

Normal aynı düzey yüz yumuşak dokusunda cilt altı yağ simetrik, masseter ve bukkal boşluk konturları düzgün, ağız tabanı yumuşak dokuları dil çevresinde dengelidir. Görüntülerde hastalık tarafı ile sağlam taraf karşılaştırılarak asimetri ve yapısal değişiklikler saptanır; kontrastlı serilerde damar dışı odaklar damar lümeniyle kıyaslanır.

Ayırıcı tanı

Yüz yumuşak doku şişliğinin ayırıcı tanısında odontojenik boşluk enfeksiyonu gibi enfeksiyöz nedenler de yer alır. Yüz yumuşak doku enfeksiyonlarında BT bulguları klinik değerlendirmeyle birlikte yorumlanmalıdır.
Yalancı anevrizma, damar duvarının kısmi yırtılmasıyla oluşan ve damar adventisyası ya da çevre yumuşak dokular tarafından sınırlanan bir hematomdur; tanıda kontrastlı BT kullanılabilir.
Traumatik parotitte parotis bezinde büyüme ve komşu yağ planlarında kirlenme görülebilir; serum alfa-amilaz düzeyleri belirgin şekilde yükselme gösterebilir.
Penetran travmalarda yara trasesi boyunca hava veya yabancı cisim fragmanları izlenebilir; bu bulgular ani havacıkların veya travmatik emfizemin de göstergesidir.

Tuzaklar

Kontrastsız BT'de görülen kanama odakları statik hematom olabilir; aktif kanama ve devam eden ekstravazasyonun ayırt edilmesi için BT anjiyografisi gereklidir.
Kontrastlı serilerde damar kesitleri ile damar dışı ekstravazyon ayrımında, ardışık kesitlerdeki devamlılık ve çok fazlı görüntüleme özellikleri değerlendirmeye dahil edilmelidir.
Ağız tabanı hematomu küçük görünse bile dili ve hava kolonunu yer değiştirebilir; sadece hematom çapına odaklanmayın.
Ciddi yüz şişliği ve travma öyküsünde, kırık hatları ve penetrant yara boyunca yabancı cisimlerin gözden kaçmaması için kemik penceresi ve yara trasesi detaylı taranmalıdır.
Kontrast ekstravazasyonunu değerlendirirken odağın anatomik olarak damar dışında olup olmadığını ve ardışık kesitlerdeki progresif değişimini inceleyin.
