Özet

Peutz–Jeghers sendromunda hamartomatöz polipler en sık ince barsakta, özellikle jejunoileal anslarda görülür ve farklı boyutlarda çok sayıda intralüminal çıkıntı oluşturabilir. İyi distandü ince barsaklı BT enterografi, poliplerin yerini, sayısını ve komşu ansla ilişkisini gösterirken akut standart kontrastlı BT invajinasyon veya obstrüksiyonu ortaya koyar. Poliplerin peristaltik baş nokta etkisi tekrarlayan invajinasyona neden olabilir; bu yüzden polip yükünü değerlendirirken ansların iç içe geçmesi, proksimal genişleme ve duvar canlılığı ayrıca aranır. Küçük polipler BT'de gözden kaçabileceğinden inceleme tek başına sendromu dışlamaz ve görüntüleme bulguları klinik/genetik bağlamla birleştirilmelidir.

Portal tanısal
İyi distandü ince barsak boyunca heterojen kontrastlanan, düzensiz veya pedinküllü polipoid çıkıntılar jejunum ve ileumda aranır; farklı segmentlerdeki çoklu odakların yerleşimi koronal reformasyonlarda izlenir. Standart portal fazlı akut BT'de iç içe ans duvarları, baş noktadaki polip, proksimal dilatasyon ve duvar boyanması komplikasyonları gösterir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Jejunum ve ileum lümenine uzanan çoklu, düzgün sınırlı yumuşak doku polipleri polipozis paternini oluşturur; lezyon sayısı ve dağılımı ardışık kesitlerde izlenir.
  • Pedinküllü polip — lümen içinde kontrastlanan baş kısmı ve barsak duvarına bağlandığı segment radyolojik olarak belirlenir.
  • Poliplerin boyut ve şekilleri aynı hastada değişebilir; büyük odaklar lümen konturunu daha belirgin bozar ve baş nokta etkisi gösterebilir.
  • İnvajinasyon — hedef biçimli iç içe barsak duvarları içinde mezenter yağı ve damarları çekilir; öndeki polip intussusceptum ucunda aranır.
  • Obstrüksiyon gelişmişse polip ya da invajinasyon düzeyinde ani kalibrasyon değişikliği, proksimal ans genişlemesi ve distal kollaps görülür.
  • İnvajine ans duvarında ödemli kalınlaşma, azalmış kontrastlanma, mezenterik konjesyon veya serbest sıvı dolaşım bozukluğu kuşkusunu artırır.
  • Aynı anda birden fazla invajinasyon odağı bulunabilir; tüm ince barsak boyunca ayrı hedef işaretleri ve ilişkili polipler taranmalıdır.
  • Yetersiz barsak distansiyonu polipleri birbirine veya duvar kıvrımlarına bindirebilir; lümen içi lezyon ile kollabe ans görünümünü ardışık ve koronal kesitlerde ayır.

Normalde

Normal, iyi distandü jejunum ve ileumda lümen boyunca ayrı, düzgün duvar kıvrımları görülür; kontrastlanan bağımsız polipoid çıkıntı ya da birbirine girmiş ans halkaları bulunmaz. Polip şüphesinde komşu normal segmentle karşılaştırma yapılmalı ve ince barsakta öncü nokta (lead point) oluşturabilecek yapılar kontrastlı görüntüleme ile taranmalıdır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Bilinen malignite öyküsü olan hastalarda ince barsak metastazları obstrüksiyona neden olabilen yumuşak doku kitlesi veya nodüler odaklar şeklinde görülebilir.
İnce barsak lenfoması polipoid görünüm yapabilir ancak genellikle segmentler arası duvar kalınlaşması ve mezenterik nodül ile ilişkilendirilir.
İnce barsak lipomu
submukozal ve homojen yağ atenüasyonlu baş noktasıdır; hamartomatöz polip kontrastlanan yumuşak doku atenüasyonundadır.
İnce barsak adenokarsinomları nadir görülür ve obstrüksiyon veya invajinasyona neden olabilir; kesin tanı ve cerrahi planlama için histopatolojik inceleme gereklidir.
İnce barsak kıvrımı veya içerik
distansiyon yetersiz olduğunda psödopolip gibi görünebilir; lümen boyunca devamlılık ve komşu düzlem görüntüsü lezyon olmadığını gösterebilir.

Tuzaklar

  • Kollabe ansın mukozal kıvrımları polipoid odak taklidi yapabilir; şüpheli yapıyı lümen distandü olduğunda takip et ve koronal/aksiyel planda duvara saplanıp saplanmadığını doğrula.
