Özet

İntestinal malrotasyon, embriyonik bağırsak rotasyonu ve fiksasyonunun tamamlanmamasıyla duodenum, jejunum ve çekumun olağandışı yerleştiği doğumsal bir durumdur. Orta bağırsak volvulusu mezenter kökünün dönmesine, venöz dönüşün bozulmasına ve hızla bağırsak iskemisine ilerleyebilir; çocukta ani safralı kusma, erişkinde ise belirsiz karın ağrısıyla ortaya çıkabilir. Çocuklarda ilk değerlendirmede genellikle ultrasonografi, gerektiğinde üst gastrointestinal kontrast incelemesi kullanılır; BT seçilmiş incelemelerde barsak dağılımını, duodenojejunal bileşkeyi, mezenterik damar ilişkisini ve eşlik eden kapalı ans/perfüzyon bozukluğunu gösterir. Volvulus ve iskemi gözden kaçarsa nekroz ve perforasyon gelişebilir; SMA-SMV ilişkisinin tek başına normal olması malrotasyonu dışlamaz.

Faz ve pencere

Portal tanısal
IV kontrast sonrası aksiyel ve koronal görüntülerde duodenojejunal bileşkenin orta hattı geçip geçmediği, jejunum ve çekumun konumu, SMV'nin SMA'ya göre yönü ve mezenterin arter çevresinde dönmesi incelenir; barsak duvarı kontrastlanması ile mezenterik venöz konjesyon da değerlendirilir.

Önerilen pencereler: Batın (G 400 / M 50), Anjiyo (G 600 / M 150).

BT bulguları

  • Duodenojejunal bileşke malpozisyonu — Dördüncü duodenum parçası orta hattı geçmez veya bileşke beklenen sol üst kadran yerleşimine ulaşmaz.
  • Sağ yerleşimli proksimal jejunum — Jejunal ansların sağ karında kümelenmesi anormal rotasyon dağılımını destekler.
  • Yüksek veya ektopik çekum — Çekumun üst karın ya da sol tarafta bulunması eşlik eden fiksasyon anomalisi olabilir.
  • SMA-SMV ilişkisi — SMV'nin SMA'nın solunda veya önünde yer alması malrotasyonu destekleyebilir; normal ilişki anomalinin yokluğunu kanıtlamaz.
  • Mezenterik girdap işareti — SMV, jejunal dallar ve mezenterik yağın SMA çevresinde spiral dönmesi volvulus şüphesini artırır.
  • Duodenal bası veya ani kalibrasyon değişimi — Ladd bantları ya da volvulus kaynaklı obstrüksiyon duodenum veya proksimal jejunumda görülebilir.
  • Mezenterik ödem/kalınma, yağ stranding'ı ve serbest sıvı gibi bulgular komplike volvulus ve iskemi şüphesini artırır.

Normalde

Normal duodenumun üçüncü parçası SMA ile aorta arasından sola geçer, duodenojejunal bileşke sol üst kadranda yer alır ve jejunum ağırlıklı olarak sol üst/orta karındadır; çekum genellikle sağ alt kadrandadır. SMA ile SMV çoğunlukla yan yana seyreder; damarların birbirine göre konumu tek başına kesin tanı vermez, bu nedenle duodenumun retroperitoneal geçişini ve tüm barsak dağılımını birlikte değerlendirin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Postoperatif iç herni
Özellikle Roux-en-Y gastrik bypass sonrası gelişen iç herniler, bağırsak tıkanıklığı yaratan sık görülen ciddi komplikasyonlardan olup ayırıcı tanıya dahil edilmelidir.
İnvajinasyon
Çocukta safralı kusma değerlendirilirken malrotasyon ve volvulusun ayırıcı tanıları arasında yer alır.
Normal SMA-SMV varyantı
Damarlar ters veya öne-arkaya konumlanabilir; bağırsak yerleşimi ve duodenumun orta hattı geçişi normalse malrotasyon kanıtı zayıftır.
Nonrotasyon paterni
İnce barsakların sağda ve kolonun solda yerleşmesi olabilir; bu, akut volvulus kanıtı olmaksızın malrotasyon spektrumunu gösterir.

Tuzaklar

  • SMA-SMV tersliği malrotasyona özgü değildir ve normal damar ilişkisi malrotasyonu dışlamaz; duodenum, jejunum ve çekum yerleşimini birlikte haritalayın.
