Özet

İntestinal tüberküloz çoğunlukla terminal ileum ve ileoçekal bölgeyi etkileyen, karın ağrısı, kilo kaybı veya sistemik enfeksiyon bulgularıyla ortaya çıkabilen ekstrapulmoner tüberkülozdur. Kontrastlı BT bağırsak duvarı tutulumunun dağılımını, mezenterik lenf nodlarını ve eşlik eden peritoneal ya da solid organ bulgularını gösterir; özellikle merkezinde düşük atenüasyon bulunan nekrotik lenf nodu Crohn hastalığından ayrımda anlamlı bir ipucudur. Görüntüleme özellikleri Crohn hastalığıyla örtüşebilir; BT bulguları intestinal tüberküloz için tek başına tanı koydurucu değildir. İleoçekal hastalık ve nodal/peritoneal yayılım fark edilmezse hastalığın kapsamı ve önemli ayırıcı tanı eksik kalabilir.

Faz ve pencere

Portal tanısal
İntravenöz kontrastlı portal venöz görüntülerde terminal ileum ve ileoçekal bileşke duvar kalınlaşması ile merkezî düşük atenüasyon ve periferik kontrastlanma gösteren nekrotik mezenterik lenf nodları aranır. Aynı fazda peritoneal kalınlaşma, asit, omental değişiklik ve karaciğer-dalak odakları değerlendirilebilir; kontrastsız görüntü nodal nekroz ve duvar kontrastlanmasını bu ölçüde karakterize etmez.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • İleoçekal duvar kalınlaşması — terminal ileum, ileoçekal valv veya çekumda duvar kalınlaşması görülebilir; tutulum Crohn ile örtüşebilir.
  • Kısa ve bitişik segment tutulumu — komşu terminal ileum ve çekumun devamlı tutulması, atlamalı uzun ileal segmentlere göre tüberkülozu daha çok destekler.
  • Nekrotik mezenterik lenf nodu — nod merkezindeki düşük atenüasyon ve çevresel kontrastlanma kazeöz nekrozla uyumludur; bu, kesitsel görüntülemede Crohn'dan ayrım için güçlü bir ipucudur.
  • Nodal kümelenme — sağ alt kadran, ileokolik damarlar çevresi veya mezenter kökünde birbirine komşu ya da birleşen lenf nodları görülebilir.
  • Peritoneal tutulum — asit, peritoneal/omental kalınlaşma veya nodülerlik karın içi tüberküloz kapsamını artırabilir.
  • Eşlik eden solid organ odakları — karaciğer veya dalakta odaklar ve toraks görüntülerinde eşlik eden tüberküloz bulguları yayılım açısından taranır.
  • Darlık ve proksimal genişleme — iyileşme-fibrozisle ilişkili ileoçekal daralma mekanik geçiş engeline yol açabilir; obstrüksiyon varlığı raporda ayrıca belirtilir.

Normalde

Normal ileoçekal bölgede terminal ileum duvarı ince ve simetriktir, ileoçekal valv çevresinde kitle benzeri kalınlaşma ve merkezî nekrozlu lenf nodu bulunmaz. Aynı seviyedeki çekum ve terminal ileumu karşılaştırırken duvar devamlılığına, valv açıklığına ve ileokolik mezenterik nodların merkezî atenüasyonuna bakın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

hacimli nodal kitle ve uzun segment duvar tutulumu yapabilir; anevrizmal lümen genişlemesi ve nodal hacim tanıyı düşündürür.

Tuzaklar

  • Nekrotik nod tüberküloz için güçlü ipucu olsa da duyarlılığı sınırlıdır; nekrotik nod görülmemesi hastalığı dışlamaz.
  • Terminal ileum kalınlaşmasını tek başına Crohn diye adlandırmayın; nekrotik nod, bitişik ileoçekal dağılım, periton ve solid organları aynı incelemede arayın.
  • Nekrotik nod görünümü enfeksiyonlar ve bazı tümörlerde de olabilir; klinik, doku örneklemesi ve mikrobiyolojiyle korelasyon gerekir.
