Özet

Uzun süreli Crohn hastalığında, özellikle kronik ileal tutulum zemininde gelişen ince barsak adenokarsinomu nadir fakat klinik sonucu önemli bir komplikasyondur. Enterografi protokolü (BT veya MR) ile duvar kalınlaşması, darlık, proksimal dilatasyon ve mezenterik uzanım incelenir. Tümörün görünümü inflamatuvar ya da fibrotik Crohn darlığını taklit edebildiğinden BT bulguları tek başına histolojik tanı değildir. Görüntüleme bulguları inflamatuar hastalıkla örtüşebildiğinden tanı gecikebilir ve cerrahi planlama zorlaşabilir.

Okuma sırası

  1. Terminal ileum. Koronal enterografi görüntülerinde kronik Crohn tutulumunun olduğu terminal ileumu ve önceki incelemeden farklı görünen kısa bir geçiş noktasını arayın.
  2. İnce barsak duvarı. Şüpheli noktada duvarın iki tarafını karşılaştırın; asimetrik kalınlık, düzensiz kontur veya fokal yumuşak doku kitlesi var mı bakın.
  3. Darlık ve proksimal ince barsak. Lümeni darlığın içinden ve ardışık anslara doğru izleyerek mekanik geçiş noktasını doğrulayın; darlık üstündeki kalibre değişimini gösterin.
  4. Mezenter ve üst batın. Son olarak mezenterik uzanım ve lenf nodlarıyla birlikte peritoneal yüzeyleri ve karaciğeri olası yayılım açısından tarayın.

Faz ve pencere

Portal tanısal
Enterografi protokolünde duvar kalınlaşması, darlık, proksimal dilatasyon, mezenterik nodlar ve ekstraintestinal tutulum incelenir. BT/MR enterografi, ince barsak duvarı, darlıklar ve ekstraintestinal tutulum hakkında kesitsel bilgi sağlar.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Fokal yumuşak doku kitlesi — kronik hastalıklı barsak segmentinde lümene veya mezenterik yüze uzanan kitle, Crohn adenokarsinomu kuşkusunu artırır.
  • Kronik tutulumda asimetrik veya eksantrik duvar kalınlaşması ve yapı kaybı malign transformasyonu düşündürebilir.
  • Kısa, belirgin darlık — uzun süreli Crohn hastalığında yeni veya ilerleyici darlık, görüntüleme maligniteyi dışlayamayabileceğinden adenokarsinom kuşkusunu gündeme getirmelidir.
  • Uzun düzensiz stenoz — Crohn zemininde stenotik segmentte malignite kuşkusu doğurabilir; klinik ve radyolojik bulgular örtüşebildiğinden bu görünüm tek başına adenokarsinom tanısı koydurmaz.
  • Proksimal ans dilatasyonu — darlığın yukarısında sıvı-gaz dolu genişlemiş anslar mekanik geçiş engelini gösterir; geçiş noktası duvar lezyonuyla eşleştirilmelidir.
  • Mezenterik yumuşak doku uzanımı — BT/MR enterografi barsak dışı tutulumun değerlendirilmesine yardımcı olur; görüntüleme bulguları tek başına adenokarsinom tanısı koydurmayabilir.
  • Lenf nodu büyüklüğü, ekstraintestinal tutulum ve uzak metastaz riski evreleme ve cerrahi planlama açısından değerlendirilir.

Normalde

Normal ince barsak duvarı incedir ve lümen serbesttir; patolojik durumlarda duvar kalınlaşması, daralma veya mezenterik tutulum gözlenebilir. Hastalıklı segmentin duvar kalınlığı, kontürü ve proksimal kalibre değişikliği normal komşu anslarla karşılaştırılarak değerlendirilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Crohn inflamatuvar darlığı
daha uzun segmentte katmanlı duvar hiperatenüasyonu, mukozal hiperemik görünüm ve comb sign gibi aktif mezenterik bulgular eşlik edebilir; yine de tümörü dışlamaz.
Crohn fibrotik darlığı
Crohn darlığının görüntüleme bulguları adenokarsinomla örtüşebileceğinden, darlık görünümü tümörü tek başına dışlamaz.
İnce barsak lenfoması
uzun segmentli, çoğu kez belirgin anevrizmal lümen genişlemesi ve hacimli nodlar oluşturabilir; adenokarsinom daha çok stenoz ve geçiş engeli yapar.
Gastrointestinal stromal tümörler (GIST)
BT özellikleri, histoloji kesinleşmeden önce ince barsak tümörü için ön tanıyı ve cerrahi planlamayı destekleyebilir.
İntestinal tüberküloz
terminal ileoçekal bölgede eşlik eden nekrotik lenf nodları veya peritoneal bulgular tanıyı destekleyebilir; Crohn zemininde görüntü örtüşebilir.
Önceki karın veya pelvis radyoterapisi
ışınlanan ince barsak segmentinde sekonder malignite olgu düzeyinde bildirilmiştir; radyoterapi öyküsü varsa kitle veya darlık bu bağlamda akılda tutulabilir. Önceki pelvik radyoterapi öyküsünde ince barsakta nadir radyasyon ilişkili angiosarkom olasılığı akla gelebilir; bu kaynak, Crohn benzeri özgül bir görüntüleme paterni tanımlamaz.

Tuzaklar

  • Bilinen Crohn darlığını otomatik olarak benign kabul etmeyin; önceki görüntülerde yeni beliren fokal asimetri, kitle veya artan proksimal dilatasyon olup olmadığını karşılaştırın.
  • Aktif inflamasyonda katmanlı hiper-kontrastlanma ve mezenterik hiperemi adenokarsinomdan daha çok Crohn aktivitesini destekler; ancak kitle görünürse ikisini birbirini dışlayan tanılar saymayın.
  • Enterografi görüntülerini sistematik olarak inceleyerek ince barsağın lümenini, duvarını ve barsak dışı bulguları değerlendirin.
  • Crohn darlığı içindeki adenokarsinom BT'de benign fibrotik veya inflamatuvar darlıktan ayırt edilemeyebilir ve görünür bir kitle oluşturmayabilir. Tek BT görünümü veya örneklemesi sınırlı negatif yüzeyel biyopsi klinik-radyolojik kuşkuyu tek başına dışlamaz.
