Özet

Radyasyon koliti ve proktiti, pelvik veya abdominal radyoterapi alan bağırsakta gelişen akut mukozal inflamasyondan kronik fibrotik hasara uzanan bir spektrumdur. BT, pelvik radyoterapi sonrası rektum ve sigmoid kolonda duvar kalınlaşmasını, mukozal kontrastlanmayı, lümen daralmasını veya striktürleri ve presakral fasya değişikliklerini gösterir. Görüntüleme bulguları özgül değildir; ışın alanı ve tedavi öyküsü olmadan nedenin radyasyon olduğu söylenemez. Kronik striktürün veya fistülün gözden kaçması obstrüksiyon ve komşu organ hasarını eksik değerlendirmeye yol açar.

Faz ve pencere

Portal tanısal
Rektum ve ışınlanan pelvik kolon duvarında ödemli kalınlaşma, mukozal kontrastlanma ve perirektal/mezorektal yağ ile fasyada inflamatuvar değişiklik aranır; kronik dönemde lümen daralması, fibrotik kalınlaşma, fistül veya koleksiyon haritalanır.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Pelvik radyoterapi alanıyla uyumlu rektum ve sigmoid kolon duvarında segmental veya daha uzun yayılımlı kalınlaşma
  • Akut inflamatuvar evrede duvar ödemi ve mukozal kontrastlanma ile birlikte çevre yağ dokusunda çizgilenme
  • Perirektal yağ hacminde artış, presakral yumuşak doku kalınlaşması veya mezorektal fasyada belirginleşme
  • Kronik hasarda düzgün ya da düzensiz lümen daralması ve fibrotik stenoz görünümü
  • Komşu mesane, vajen veya ince bağırsakla kolon arasında fistül yolu ya da enterik içerik/gaz geçişi
  • Darlığın proksimalinde kolon veya ince bağırsak anslarında distansiyon ve sıvı-gaz birikimi
  • Duvar bütünlüğü bozulduğunda perirektal koleksiyon veya ekstraluminal hava

Normalde

Normal rektum, mezorektal yağla çevrili düzgün bir duvar ve açık lümen olarak izlenir; presakral fasya ince, çevre yağ planları seçilebilirdir. Tedavi alanındaki anormal segmenti daha yukarıdaki kolonla kıyaslayıp duvar kalınlığı, lümen çapı, mezorektal fasya ve komşu pelvik organlarla arasındaki planların korunup korunmadığına bak.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Radyasyon alanındaki benign fibrozis ve proktit difüzyon kısıtlaması gösterebilir; nüks tümörden ayırt etmek için duvar morfolojisi ve kontrastlanma örüntüsü değerlendirilmelidir.
  • Presakral kalınlaşma ve perirektal çizgilenme tedavi sonrası tek başına aktif kolit kanıtı değildir; tedavi zamanı ve seri görüntüler önemlidir.
  • Fistül traktı tek bir eksende komşu yapılarla karışabilir; traktın tam seyri ve komşu organ ilişkisi çoklu plane görüntülemelerle daha güvenilir haritalanır.

    Rektovezikal fistül. Trakt, komşu organlar arasında iletişim kurduğunu gösterir ve fistülü tanımlar (S62). Divertikülit en sık sigmoid kolonda görülür; görüntülemede divertiküllere duvar kalınlaşması ve perikolik yağ dokusunda çizgilenme eşlik edebilir. Fistül değerlendirmesinde traktın seyri ve eşlik eden inflamatuvar değişiklikler, koleksiyon/apse veya pelvik organ tutulumu aranır. Kolovezikal fistülde mesane duvarını tutan traktla birlikte mesane lümeninde hava veya debris görülebilir.
