Özet

Gastrik divertikül, çoğunlukla fundusun arka duvarından dışarı uzanan ve rastlantısal saptanan nadir bir keseleşmedir. BT'de sol diyafram krusu, dalak ve sol adrenal bez arasındaki küçük hava-sıvı içerikli yapı, mide lümeniyle boynu gösterilebildiğinde tanınır. Bu devamlılık kurulmazsa divertikül adrenal kitle, aksesuar dalak veya pankreas kuyruğu komşuluğundaki kistik lezyon sanılabilir. İnflamasyon ya da perforasyon bulgusu varsa sıradan anatomik varyant olarak raporlanması klinik açıdan önemli komplikasyonu atlatabilir.

Faz ve pencere

Kontrastsız tanısal
Kese içinde hava, sıvı veya hava-sıvı seviyesi; ince duvarı ve mide fundusuna uzanan boynu gösterilebilir. Kesedeki hava-sıvı seviyesi ve ince duvar tipiktir; mide lümeniyle bağlantının gösterilmesinde oral kontrastlı BT yardımcı olabilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Tipik kese, gastroözofageal bileşkenin hemen altında fundusun posterior yüzünden dışa uzanır.
  • Kese sol diyafram krusu ile dalak ve sol adrenal bez arasında yerleşebilir.
  • İçerikte hava, sıvı veya ikisinin oluşturduğu seviye bulunabilir.
  • Mide lümeniyle ince boyun ya da doğrudan devamlılık, divertikül lehine en kullanışlı anatomik kanıttır.
  • Divertikül içinde kronik gastirit gelişebilir; perforasyon ve kanama gibi ciddi komplikasyonlar da görülebilir.
  • Sol adrenal bezle karışıklık yaratabilir; adrenal bezin kendi yapısını koruması ayırıcı tanıda yardımcı olur.

Normalde

Normal fundus posterior duvarı düzgün konturludur ve arkasındaki sol krus, adrenal bez ile dalak arasında ayrı bir kese oluşturmaz. Şüpheli oluşumu aynı düzeyde fundusla kesit kesit takip et; lümenle boyun ilişkisini, içindeki gaz/sıvıyı ve komşu organların kendi konturunu koruyup korumadığını karşılaştır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Tek kesitte kistik adrenal kitleyi divertikül sanma; koronal-sagittal planda fundus lümenine açılan boynu göster.
  • Küçük veya sıvı dolu divertikülde gaz görülmeyebilir; yalnız hava yokluğuyla tanıyı dışlama.
  • Divertikül çevresindeki yağ planı inflamasyon yoksa temizdir; kalın duvar ve yağ kirlenmesi varsa komplikasyon olasılığını ayrıca belirt.
