Özet

Gastrektomi geçirmiş mide kanseri hastasında yeni karın yakınmaları, tümör belirteci artışı veya takip incelemesi sırasında lokal nüks ve peritoneal metastaz araştırılır. BT, anastomoz çevresi ve cerrahi yataktaki nüksü değerlendirmenin yanı sıra peritoneal nodülleri, asiti, lenf nodlarını ve karaciğer metastazlarını göstermeye yardımcı olur. Ameliyat sonrası değişiklikler tümör nüksünü taklit edebildiğinden, BT bulgularının yorumlanması güçleşebilir. BT, küçük hacimli peritoneal karsinomatozun gösterilmesinde daha az duyarlı olabilir; bu hastalık gözden kaçarsa yayılım olduğundan az değerlendirilebilir.

Okuma sırası

  1. gastrojejunal veya özofagojejunal anastomoz. Lokal-bölgesel nüks açısından cerrahi yatağı ve anastomoz çevresini değerlendir.

BT bulguları

  • Omental kekleşme ya da yaygın retiküler omental kalınlaşma
  • İnce bağırsak ansları arasında veya pelviste yeni, açıklanamayan serbest sıvı
  • Karaciğerde yeni hipodens metastatik odak veya büyüyen bölgesel/uzak lenf nodu
  • İnce bağırsak serozası ve mezenter boyunca peritoneal nodüller aranmalıdır; BT küçük hacimli karsinomatozun gösterilmesinde daha az duyarlı olabilir.

Normalde

Aynı rezeksiyon tipinden sonra normal BT'de mide rezeksiyonuna bağlı yeniden düzenlenmiş anatomi ve anastomoz çevresindeki ince, düzgün cerrahi skar beklenebilir; bu görünüm önceki incelemelerde genellikle kararlıdır. Kıyaslamada özellikle anastomoz lümeninin devamlılığı, cerrahi yatak konturu, peritoneal yüzeyler ve omental yağın yeni nodülerleşme gösterip göstermediği incelenir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
