Özet

Kopuk pankreatik kanal sendromu, çoğunlukla nekrotizan pankreatitte kanalın nekrozla kesintiye uğraması ve kuyruk tarafındaki canlı pankreasın duodenuma drenajını kaybetmesiyle gelişir. Kontrastlı BT nekrotik boyun/gövde segmentini, canlı distal parankimi ve tekrarlayan koleksiyonları haritalar; kanal devamlılığını doğrudan göstermek için MRCP veya pankreatografi gerekebilir. Persistan koleksiyon veya dış fistülün nedenini açıklamak ve drenajın tekrarlama riskini fark etmek için bu anatominin tanınması önemlidir. Kuyruk tarafındaki canlı segment gözden kaçarsa, kopuk pankreatik kanal sendromu tanısı gecikebilir; bu durum yinelenen koleksiyonlar, dış fistül veya tekrarlayan pankreatitle ortaya çıkabilir.

Portal tanısal
Kontrastlı BT nekrotik pankreas alanını ve koleksiyonları değerlendirebilir, kuyruk tarafındaki canlı pankreas dokusunu gösterebilir; kanal bütünlüğünü doğrulamak için MRCP veya ERCP gerekebilir. İlk iki haftada pankreatik kanal bütünlüğündeki bozulmanın gelişip gelişmeyeceği genellikle belirsizdir.
Arteriyel tanısal
Pankreatik parankim fazında boyun-gövde nekrozunun tüm kalınlığı tutup tutmadığı ve kuyruk parankiminin canlı kontrastlanması karşılaştırılır; bu faz kanal lümenindeki kesintiyi tek başına doğrulamaz.

  • Pankreas boyun/gövdesinde en az 2 cm'lik nekroz alanı
  • Nekrozun kuyruk tarafında korunmuş kontrastlanan distal pankreas parankimi
  • Nekrotizan pankreatit sonrası akut nekrotik koleksiyon veya duvarlı nekroz gelişmesi
  • Koleksiyonun rezolüsyonundan sonra distal ana kanalın ani sonlanması veya dilate distal kanal
  • Kontrastlı BT nekrotik alanı ve canlı distal pankreası gösterebilir; kopuk kanal tanısını doğrulamak için çoğu kez MRCP veya ERCP ile kanal anatomisinin değerlendirilmesi gerekir.
  • Aynı bölgede büyüyen/tekrarlayan psödokist veya nekrotik koleksiyon
  • Peripankreatik koleksiyonun anterior pararenal alana uzanması
  • Kopuk kanal sendromunu düşündüren temel bulgular, en az 2 cm'lik pankreatik nekroz alanının distalinde canlı pankreas dokusu bulunması ve ERCP'de ana kanalın tam kesilmesi ya da kontrast ekstravazasyonudur.

Ölçütler ve sınıflamalar

Kopuk kanal tanı ölçütlerinde nekroz uzunluğu
Tam kat nekroz alanı (genellikle ≥2 cm) ve bu alanın kuyruk tarafında canlı pankreas dokusu kopuk kanal şüphesini destekler; kanal anatomisi MRCP veya pankreatografiyle değerlendirilir, pankreatografide tam kesilme ya da kontrast ekstravazasyonu tanıyı destekler.

Normalde

Kontrastlı BT'de korunmuş pankreas parankimi kontrastlanır; nekroz alanlarında ise kontrastlanma yamalı veya geniş biçimde azalır ya da kaybolur. Kontrastlı BT'de nekrotik pankreas alanını ve koleksiyonları değerlendir; nekrozun kuyruk tarafındaki canlı pankreas dokusunu da incele.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Ana pankreatik kanal darlığı, özellikle nekroz alanı 2 cm'den küçük olduğunda iyileşme sonrası gelişebilir ve eksik kopuklukla ilişkili olabilir.
Pankreatik psödokist, interstisyel ödematöz pankreatit sonrası gelişen olgun bir koleksiyondur; nekrotizan pankreatit sonrasında koleksiyonla ilişkili canlı distal pankreas ve kanal kesintisi saptanması kopuk pankreatik kanal sendromunu düşündürür.
Duvarlı nekroz, heterojen ve nekrotik artıklar içerebilen bir koleksiyondur; pankreatik kanal kopukluğu buna eşlik edebilir.
Kronik pankreatit, pankreas içinde ilerleyici fibrozis, hücresel infiltrasyon ve parankim kaybıyla karakterizedir.

Tuzaklar

  • Kontrastlı BT kanal kesintisini doğrudan göstermekte yetersiz kalabilir; kopuk pankreatik kanal sendromunu doğrulamak için MRCP veya ERCP ile kanal anatomisi değerlendirilebilir.
  • Akut nekrozun erken evresinde perfüzyon kaybı ve koleksiyon sınırları evrim gösterebilir; tek erken inceleme sonraki kopukluğu dışlamaz.
  • Koleksiyonun küçülmesi kanal sorununu çözülmüş göstermez; aynı bölgede yeniden sıvı birikmesi distal bez sekresyonunun sürdüğünü düşündürür.
