Özet

Ektopik pankreas, normal pankreasla anatomik veya damarsal bağlantısı bulunmayan pankreas dokusudur; mide antrumu ve proksimal duodenum sık yerleşim alanlarıdır. Çoğu tesadüfi submukozal nodül olarak görülür, fakat görünümü gastrointestinal stromal tümör gibi duvar kaynaklı lezyonları taklit edebilir. Portal venöz fazlı BT, duvar içindeki nodülün konumunu ve kontrastlanmasının ortotopik pankreasla benzerliğini karşılaştırmaya yarar. Normal bezden ayrı yerleşim ve olası komplikasyonlar tanımlanmazsa kitle yanlış sınıflanabilir; BT görünümü tek başına her olguda kesin tanı koydurmaz.

Faz ve pencere

Portal tanısal
Mide antrumu veya proksimal duodenum duvarında yerleşen yuvarlak-oval nodül kontrastlanır; dansite ve kontrastlanma örüntüsü normal pankreasa benzeyebilir, ancak sınırları çoğunlukla belirsizdir. Duvar merkezi ve ortotopik pankreastan ayrı oluşu değerlendirilir.

Önerilen pencereler: Batın (G 400 / M 50).

BT bulguları

  • Mide antrumunun prepilorik bölümünde, sıklıkla büyük kurvatur tarafında, duvar içi veya submukozal nodül yerleşir.
  • Proksimal duodenum duvarında benzer, genellikle sınırları belirsiz oval yumuşak doku odağı görülebilir.
  • Nodül ortotopik pankreastan ayrı konumdadır ve arada doğrudan parankim devamlılığı bulunmaz.
  • Lezyonun kontrastlanma dansitesi ve örüntüsü normal pankreas dokusuna benzerlik gösterebilir.
  • Üzerindeki mukozada nadiren santral çöküklük veya hafif mukozal uyarılma görülebilir; BT'de her olguda seçilemez.
  • Rudimenter duktusa bağlı santral çöküklük endoskopide görülebilir; rutin BT'de duktal bağlantı çoğunlukla seçilemez.
  • İnflamasyon gelişmişse lezyon çevresinde duvar kalınlaşması, ödem veya kistik değişiklik eşlik edebilir.

Normalde

Normal pankreas üst batında lobüle parankim olarak yer alır ve mide antrumu ile duodenum duvarından ayrıdır. Aynı düzeyde lezyonun duvarın hangi katında/kenarında durduğunu, ortotopik pankreastan aradaki yağ planını ve nodül kontrastlanmasının pankreas parankimine benzer olup olmadığını karşılaştır; mukozal yüzey tek başına BT ile güvenle değerlendirilemez.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Gastrointestinal stromal tümör
duvar merkezli subepitelyal kitle olabilir; pankreas benzeri kontrastlanma ve prepilorik/duodenal yerleşim ektopiyi destekler ama tek başına ayırıcı değildir.
Leiomyom
endoskopik ultrasonografide genellikle muscularis propria tabakasından kaynaklanan, düzgün sınırlı ve homojen hipoekoik bir lezyondur; BT'de pankreas benzeri kontrastlanma göstermediğine dair doğrudan veri yoktur.
Nöroendokrin tümörler mide subepitelyal lezyonlarının ayırıcı tanısında yer alır.
Duodenal adenokarsinom
düzensiz duvar kalınlaşması, lümen daralması ve infiltratif sınırlar beklenir.
Duplikasyon kisti
sıvı dansitesinde, çoğunlukla kontrastlanmayan kistik lezyondur; solid pankreas benzeri doku değildir.

Tuzaklar

  • Küçük submukozal nodül BT'de GIST ile ayırt edilemeyebilir; pankreas benzeri kontrastlanma ancak destekleyici bulgudur.
  • Kısmi hacim etkisi lezyon ile ortotopik pankreas arasında devamlılık izlenimi verebilir; ardışık kesit ve koronal düzlemde ayrı yerleşimi doğrula.
  • Santral çöküklüğün her görüntülemede görünmesini bekleme; BT'de yokluğu ektopik pankreası dışlamaz.
  • Ektopik pankreasın kendisi de inflamasyon veya kistik değişiklik gösterebilir; duvar bulgusunu komşu organla sınırlı sanma.
