Özet

Karın duvarı selüliti, derinin dermis tabakası ve deri altı yumuşak dokularını tutan bakteriyel bir enfeksiyondur; tanı kliniğe dayanır ve altın standart laboratuvar testi yoktur. Tanı öncelikle klinik bulgulara dayanır; BT, yumuşak doku enfeksiyonlarının değerlendirilmesine ve derin yayılım bulgularının araştırılmasına yardımcı olabilir. Kontrastlı incelemede yaygın cilt kalınlaşması ve subkutan yağ dokuda ödem ön plandadır, fakat sınırlı koleksiyon olmaması enfeksiyonu dışlamaz. Fasyal gaz, derin sıvı veya kas tutulumu gözden kaçarsa daha ağır bir yumuşak doku enfeksiyonunun yayılımı eksik değerlendirilir.

Görüntülemede yaygın karın duvarı ödemi görülebilir; bu bulgu tek başına selülit tanısını doğrulamaz. Selülit diffüz inflamasyon ile karakterizedir; apse ise periferik kontrastlanma ve sınırlı koleksiyon ile ayrılır.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Batın (G 400 / M 50).

BT bulguları

  • Enfekte bölgede deri kalınlaşması ve diffüz ödem izlenir; bazı durumlarda cilt yüzeyinde peau d'orange benzeri görünüm gelişebilir.
  • Karın duvarı selüliti şüphesinde görüntülemede yaygın duvar ödemi görülebilir; bu görünüm tek başına bakteriyel enfeksiyonu kanıtlamaz.
  • Yumuşak doku enfeksiyonları deri ve subkutan dokunun ötesine geçerek fasya ve kas katmanlarına da yayılabilir.
  • Sınırlı, periferik kontrastlanan sıvı koleksiyonu oluşumu yumuşak doku apsesini düşündürür.
  • Karın duvarı selüliti cerrahi kesi sonrası görülebilir.
  • Fasya boyunca sıvı, gaz ve derin doku tutulumu nekrotizan yumuşak doku enfeksiyonu veya miyosit kuşkusu açısından kritik bulgulardır.

Etkilenen alanda deri altı ödemin yanı sıra fasya boyunca sıvı veya gaz ve derin doku yayılımı bulgularını da inceleyin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Subkutan apse, periferik kontrastlanma ve sınırlı sıvı koleksiyonu ile karakterizedir; selülit ise diffüz inflamatuar değişikliklerle seyreder.
Nekrotizan enfeksiyonlar fasya boyunca gaz, sıvı ve ödemle derin kompartımanlara hızlı yayılım gösterir.
Rektus kası içindeki hematom, BT'de 50–75 HU yüksek atenüasyon gösterebilir.
Miyosit ve intramüsküler apse durumlarında kas içi ödem ve enfeksiyon odakları belirgindir.
Postoperatif dönemde oluşan koleksiyonların benign fibrotik reaksiyon (tekstiloma) ile akut apse arasındaki ayrımı radyolojik özelliklere ve klinik tabloya dayanır.

Tuzaklar

  • Deri altı ödem tek başına bakteriyel selülite özgü değildir; bağımlı ödem, venöz tıkanma ve ameliyat sonrası değişikliklerde de görülebilir, karşı tarafı ve klinik dağılımı kıyaslayın.
  • Erken dönem görüntülemede apse formasyonu tam olarak ortaya çıkmayabilir; takipte kontrastlanma paternindeki değişiklikler dikkatle izlenmelidir.
  • Fasya ve kas kompartımanlarına uzanan sıvı/gaz varlığı, enfeksiyonun derin dokulara yayıldığını gösterir ve ciddi enfeksiyon riskini artırır.
