Özet

Mezenterik yağ çizgilenmesi ve sıvı dansitesi, akut bağırsak iskemi veya obstrüksiyon gibi çeşitli durumlarda görülen özgül olmayan BT bulgularıdır; tek başına spesifik bir etiyolojiye işaret etmez. Portal venöz fazlı BT, mezenterik venöz dönüşü, bağırsak duvarını ve eşlik eden asit ya da sistemik konjesyon işaretlerini birlikte değerlendirmeyi sağlar. Yağ halkası ve hiperdens psödokapsül mezenterik pannükülitin tipik BT bulguları arasındadır; mezenterik yağdaki puslu atenüasyon artışının ayırıcı tanısında mezenterik ödem ve lenfoma gibi nedenler de bulunur. Bulguyu yalnız “ödem” diye adlandırıp altta yatan venöz tromboz, iskemi veya kalp/karaciğer kaynaklı konjesyonu gözden kaçırmak klinik önemini azaltabilir.

Faz ve pencere

Portal tanısal
Portal venöz kontrastlanmada mezenterik yağ lobüllerinde yaygın veya damar çevresinde ince çizgilenme ve sıvı dansitesi araştırılır; aynı fazda superior mezenterik ven/portal ven açıklığı, bağırsak duvarı kontrastlanması, asit ve sistemik ödem eşlikçileri değerlendirilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Mezenterik yağın normal homojen düşük atenüasyonunun yerini alan yaygın puslu görünüm ve ince retiküler çizgilenme (misty mesentery).
  • Mezenterik arter ve ven dallarını saran sıvı/ödem yoğunluğunda kılıflanma; damarların lümen açıklığı ayrıca izlenmelidir.
  • Mezenterik yağ çizgilenmesi ve mezenterik sıvı akut bağırsak iskemisinde görülebilir; bunlar özgül olmayan bulgulardır ve varlıkları iskeminin nedenini tek başına belirlemez.
  • Venöz konjesyonda genişlemiş mezenterik venler, duvar kalınlaşması ve submukozal ödem; trombüs varsa ven lümeninde dolum defekti.
  • İskemi eşlik ediyorsa bölgesel duvar kontrastlanma bozukluğu, pnömatozis veya mezenterik sıvı görülebilir; yağ çizgilenmesi tek başına iskemiye özgü değildir.
  • Sistemik sıvı yükünde mezenterik değişikliklere asit, cilt altı ödemi, plevral efüzyon veya kardiyomegali eşlik edebilir.
  • Fokal, kitle benzeri yağ infiltrasyonu, çevreleyen hiperdens psödokapsül veya yağ halkası görünümü ödemden çok mezenterik pannüküliti düşündürür.
  • Misty mesentery görünümünün ayırıcı tanısında mezenterik ödem ve lenfoma gibi hastalıklar yer alır; görünüm tek başına nedeni belirlemez.

Normalde

Normal mezenter kökünde yağ lobülleri homojen ve düşük atenüasyonludur; jejunal damarlar çevresinde sıvı kılıfı ya da yapraklar arasında çizgisel sıvı görülmez. Aynı düzeyde damar çevresi yağ planlarının keskinliğini, superior mezenterik ven kalibresini ve komşu bağırsak duvarı ile peritoneal sıvı miktarını kıyasla; ödemde değişiklik bu normal yağ-damar kontrastını bulanıklaştırır. Mezenterik pannükülitte mezenterik damarları çevreleyen korunmuş hipodens yağ halkası görülebilir; bu bulgu mezenterik yağdaki atenüasyon artışının nedenini tek başına belirlemez.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Mezenterik pannükülit
çoğunlukla ince bağırsak mezenter kökünde fokal yağ atenüasyonu artışı, yağ halkası ve/veya psödokapsülle sınırlı kitle benzeri görünüm oluşturur.
Lenfoma
kümelenmiş, büyümüş mezenterik lenf düğümleri ve bazen damarları çevreleyen yumuşak doku kitlesi öne çıkar; yaygın ödemde kitle/nodal yük beklenmez.
Mezenterik ven trombozu
portal fazda SMV veya dallarında intraluminal dolum defekti; drenaj alanındaki duvar ödemi ve mezenterik sıvı venöz iskemiye eşlik edebilir.
Arteriyel mezenterik iskemi
arteriyel oklüzyon veya belirgin darlık ve etkilenen bağırsak segmentinde kontrastlanma bozukluğu aranır; mezenterik yağ pusluluğu özgül değildir.
Peritoneal karsinomatozis
mezenterik ödemden farklı olarak nodüler peritoneal/omental kalınlaşma, implantlar ve sıklıkla asit görülebilir.
Komşu bağırsak inflamasyonu
yağ çizgilenmesi belirli bir hastalıklı bağırsak segmentine odaklanır ve duvar kalınlaşması ya da mukozal hiperemiyle birlikte bulunur.
Misty mesentery görünümünün ayırıcı tanısında mezenterik ödem ve lenfoma gibi hastalıklar yer alır; nodal ve kitle bulguları ayırıcı değerlendirmede önem taşır.

Tuzaklar

  • “Misty mesentery” veya hafif bulanıklık, mezenterik pannükülitten ziyade sistemik sıvı yükü kaynaklı yaygın mezenterik ödem ile uyumlu olabilir; lokalizasyon, lenf nodülleri ve damar tutulumu ayrımı sağlar.
  • Mezenterik yağ çizgilenmesi venöz konjesyon ve inflamasyonda da görülür; yalnız bu işarete dayanarak bağırsak iskemisi tanısı koyma.
  • Düşük hacimli venöz trombüs kontrastlı kesitlerde gözden kaçabilir; SMV'yi mezenter kökünden portal ven birleşimine kadar kesitler ve çok düzlemli görüntülerde izle.
  • Asit ve yaygın ödem kalp yetmezliği ya da portal hipertansiyonla ilişkili olabilir; mezenterdeki değişikliği tek başına primer inflamatuvar lezyon gibi raporlama.
