Özet

Kaburga kırıkları BT’de farklı iyileşme evrelerinde görülebilir; semptomatik kaynamama, kırık yerinde süren ağrıyla ilişkili olabilir. BT'de kallus oluşumu, köprüleşen trabeküller ve kırık hattındaki küçülme iyileşme değerlendirmesine katkı sağlar; görüntülemeyle kırık yaşını belirlerken temkinli olunmalıdır. BT'de köprüleşmeyen kallus, özellikle yaralanmadan en az 3 ay sonra kırık yerinde ağrı varsa, semptomatik kaburga kaynamamasıyla uyumlu olabilir; tanım klinik belirti ve süreyi de içerir. Kırık yaşının yanlış belirlenmesi, yaralanmanın belirli bir olayla ilişkilendirilmesini ve adli değerlendirmeyi etkileyebilir.

Faz ve pencere

Kontrastsız tanısal
BT'de köprüleşmeyen kallus ve kırık hattı izlenir; iyileşme süreci klinik bulgular ve önceki görüntülerle karşılaştırılarak değerlendirilir. Kemik iyileşmesi değerlendirilirken öncelik aksiyel ve çok düzlemli tomografik rekonstrüksiyonlardır; klinik ve radyolojik bulgular bir arada yorumlanır.
Kemik algoritması tanısal
Aksiyel ve çok düzlemli kemik rekonstrüksiyonları köprüleşen kallusu, remodelasyonu, kaynamama hattını ve fragmanların hizasını gösterir; önceki inceleme varsa değişim ve yeni kırık hattı karşılaştırılır.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Periostal kallus, iyileşme sürecinin tamir fazında görülen ve radyolojik olarak izlenen önemli bir bulgudur.
  • Kırık bölgesinde remodelasyon, kemik iyileşmesinin zaman içindeki evrelerinden biridir.
  • Kırık hattında köprüleşme — karşılıklı kemik uçları arasında devamlı yeni kemik görülmesi kaynama lehinedir.
  • Kalıcı kırık aralığı — BT'de köprüleşmeyen kallus, özellikle yaralanmadan en az 3 ay sonra kırık yerinde ağrı varsa, semptomatik kaynamamayı düşündürebilir.
  • Kırıkta deformite veya hizasızlık göğüs duvarı stabilitesini bozabilir ve cerrahi stabilizasyon gerekliliğini gündeme getirir.
  • Hipertrofik kaynamama — belirgin kallus oluşmasına rağmen fragmanlar arasında köprüleşmenin ve kemik devamlılığının olmaması.
  • Kemik kaynamasında gecikme veya başarısızlık durumunda, radyolojik bulgular klinik semptomlar ve geçen süre ile birlikte değerlendirilmelidir.
  • Komşu yumuşak doku — belirgin kitle, kemik destrüksiyonu veya ilerleyici kortikal erozyon basit iyileşen kırık için tipik değildir.

Normalde

İyileşme bulgusunun çevresinde kortikal yıkım varsa agresif kemik lezyonu açısından ileri patolojik değerlendirme gerekir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Primera kemik lezyonları veya enfeksiyonlar, travmatik kallus görünümünü taklit edebilir ve altta yatan patolojiyi araştırmak için biyopsi gerekebilir.
Heterotopik ossifikasyon
kaburga dışındaki yumuşak dokuda kemikleşme odakları görülür; kosta korteksine bağlı kırık iyileşmesi değildir.
Kronik kaburga osteomiyeliti BT'de kaburgaya invazyon gösteren bir göğüs duvarı kitlesi şeklinde görülebilir.

Tuzaklar

  • Kallus varlığından takvim günü çıkarmayın; iyileşme hızı yaş, kemik ve yaralanma özelliklerine göre değişir, BT bulgusu kesin tarih vermez.
  • Kallus çevresindeki keskin hattı otomatik olarak yeni kırık saymayın; önceki BT, yeni travma öyküsü ve fragman hizasındaki değişimle karşılaştırın.
