Özet

Göğüs duvarında hızla ilerleyen ağrı, sistemik toksisite veya cilt değişikliği bulunan hastada enfeksiyon cilt altından derin fasyaya ve kaslara yayılabilir. Kontrastlı BT; gazın dağılımını, fasyal sıvıyı, dokuların kontrastlanmasını ve plevra ya da mediastene uzanımı aynı incelemede haritalar. Gaz görülmemesi erken veya gaz oluşturmayan nekrotizan enfeksiyonu dışlamaz; tanı yalnız BT bulgusuna bağlanmamalıdır. Yayılımın ve derin doku nekrozunun atlanması hastalık sınırlarının eksik değerlendirilmesine yol açabilir.

Portal tanısal

Yumuşak doku penceresinde cilt altı yağ dokusu, yüzeyel ve derin fasya ile kasların kalınlığı ve kontrastlanması izlenir; fasyal sıvı, perifasyal gaz, fasyada azalmış kontrastlanma ve çevreye yayılan koleksiyon aranır. BT, hastalığın yayılımını ve plevral ya da mediastinal uzanımını gösterebilir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Mediasten (G 350 / M 50), Akciğer (G 1500 / M -600).

BT bulguları

• Derin fasya boyunca sıvı — kas kompartımanını izleyen ince veya yaygın sıvı, tek başına gazdan daha uyarıcı olabilir.
• Fasyal gaz — yüzeyel fasya, interkostal planlar veya kaslar boyunca lineer ve dallanan hava odakları.
• Fasya kalınlaşması ve kontrastlanma kaybı — nekroz olasılığını artırır; tek başına cerrahi nekroz kanıtı değildir.
• Yaygın asimetrik yağ dokusu dansite artışı — cilt altı dokudan derin kompartımanlara uzanan inflamatuvar çizgilenme.
• Kas tutulumu — pektoralis veya interkostal kaslarda şişme, heterojen kontrastlanma ya da nekrotik alan.
• Doku içi hava-sıvı koleksiyonu — enfekte kavitenin cilt altı dokudan aksilla, plevra veya mediastene uzanması.
• Komşu plevral/mediastinal devamlılık — ampiyem, plevral kalınlaşma veya mediastinal inflamasyonla birlikte izlenebilir.

Normalde

Aynı seviyenin karşı tarafında kas kalınlığını, yağ dokusu çizgilenmesini ve plevral yüzeyi kıyaslayın; yaygın simetrik ödem enfeksiyon lehine asimetrik kompartıman yayılımından farklıdır.

Göğüs duvarı apsesi

çevresel kontrastlanan sınırlı koleksiyon ön plandadır; çevre fasyalarda yaygın kalınlaşma nekrotizan yayılımı düşündürür.

Cerrahi veya travmatik amfizem

hava cilt altı ya da kas planlarını izleyebilir; eşlik eden belirgin asimetrik fasyal ödem ve nekrotik kontrastlanma yoksa enfeksiyon daha az olasıdır.

Enfekte hematom, göğüs duvarı nekrotizan enfeksiyonunun olası kaynaklarından biri olabilir.

çoğunlukla yaygın ve iki taraflı cilt altı ödemidir; derin fasya boyunca odaksal gaz beklenmez.

Tuzaklar

• Gaz yokluğunu dışlama bulgusu saymayın; erken dönemde veya gaz oluşturmayan enfeksiyonda fasyal sıvı ve kontrastlanma bozukluğu daha görünür olabilir.
• Enfeksiyonla aynı taraftaki dren veya ameliyat hattı hava oluşturabilir; hava dağılımını cihaz yoluna ve yumuşak doku inflamasyonuna göre izleyin.
