Retikülasyon ve bal peteği: fibrozis paternleri BT bulguları

Özet

Bal peteği, çoğunlukla subplevral akciğerde kümelenen kistik hava boşluklarıyla yerleşik fibrozisi gösterir. Bu ayrım özellikle olağan interstisyel pnömeni (UIP) paternini değerlendirirken kritiktir; yüksek güvenirlikli BT'de UIP paterninin saptanması cerrahi biyopsi gerektirmeden tanıyı destekler. İnce kesitli BT dağılımı ve özelliklerinin karakterizasyonu, ayırt edici radyolojik paternler oluşturarak tanısal süreci yönlendirir. Bal peteğini amfizem veya traksiyon bronşiolektazisi sanmak UIP paterninin yanlış sınıflanmasına yol açabilir.

Faz ve pencere

Kontrastsız tanısal: İnspirasyonda alınan kontrastsız ince kesit HRCT'de retiküler çizgiler, subplevral kistik boşluklar ve traksiyonla genişlemiş hava yolları akciğer penceresinde seçilir; kontrast fazı gerekmez.

Önerilen pencereler: Akciğer (G 1500 / M -600).

BT bulguları

İntralobüler retikülasyon, kesişen ince çizgilerle düzensiz ağ görünümü oluşturur.
İnterlobüler septal kalınlaşma retikülasyona eşlik edebilir; düzgün veya düzensiz dağılımı altta yatan süreci tek başına belirlemez.
Bal peteği, çoğunlukla subplevral yerleşimli, belirgin duvarlı ve benzer çaplı kümelenmiş kistik hava boşluklarıdır.
Bal peteği boşlukları yan yana ve birden fazla katman halinde izlenebilir; erken olguda tek subplevral sıra da görülebilir.
Bazal ve periferik baskınlık UIP paternini destekler; yamalı dağılım eşlik edebilir.
Traksiyon bronşektazisi ve bronşiolektazisi, fibrotik zeminde düzensiz genişlemiş ve çekintiye uğramış hava yolları şeklindedir.
Lob hacminde azalma, fissür ve bronkovasküler yapıların çekilmesi mimari distorsiyonu gösterir.

Ölçütler ve sınıflamalar

Bal peteği kistlerinin tipik çapı: 3–10 mm çapında kümelenmiş kistik hava boşluklarıdır.
UIP paterni: Subplevral ve bazal baskın retikülasyon ile bal peteği vardır; traksiyon bronşektazisi eşlik edebilir. Alternatif tanıyı düşündüren BT bulgusu yoktur.
Olası UIP paterni: Subplevral ve bazal baskın retikülasyon ile traksiyon bronşektazisi veya bronşiolektazisi vardır; bal peteği ve alternatif tanıyı düşündüren bulgu yoktur.
UIP açısından belirsiz: Fibrozis bulguları bulunur ancak BT, UIP veya olası UIP ölçütlerini karşılamaz ve belirgin bir alternatif tanıyı da düşündürmez.
Alternatif tanı düşündüren BT bulguları: Üst/orta zon veya peribronkovasküler baskınlık, yaygın buzlu cam, belirgin mozaik atenüasyon ya da hava hapsi, çok sayıda mikronodül veya konsolidasyon UIP dışı tanıyı düşündürür.

Normalde

Normal akciğerde sekonder lobüllerin sınırları çoğu yerde seçilmez; bazal veya apikal periferide seyrek, çok ince septal çizgiler görülebilir. Aynı düzeyde plevraya komşu çizgilerin kalıcı ağ oluşturup oluşturmadığını, kistik hava boşluklarının duvar ve katmanlarını ve bronşların çevre fibrozisle çekilip çekilmediğini karşılaştır.

Normal BT ile kıyasla

Ayırıcı tanı

Paraseptal amfizem: duvarı ince ve belirsiz düşük atenüasyon alanları çoğunlukla tek subplevral sıra yapar ve çevrede fibrotik distorsiyon bulunmaz.
Traksiyon bronşiolektazisi: dallanan veya ardışık kesitlerde devam eden hava yolu lümeni bronş ağacıyla bağlantılıdır.
Fibrotik NSIP: daha düzgün ve yaygın retikülasyonla birlikte bazal subplevral korunma görülebilir; bal peteği UIP'ye göre daha az tipiktir.
Bağımlı atelektazi: sırtüstü kesitlerde posterior bazalde dansite artar, yüzüstü kesitte belirgin gerileyebilir.
Ödem: düzgün septal kalınlaşmaya damar belirginliği ve plevral sıvı eşlik edebilir; kistik bal peteği yapısı oluşturmaz.

Tuzaklar

Tek sıra subplevral kist otomatik olarak bal peteği değildir; duvarların belirginliği ve eşlik eden retikülasyon ile mimari çekintiyi birlikte ara.
Uçtan görülen traksiyon bronşiolektazisi, fibrotik zeminde duvarı kalınlaşmış ve lümeni genişlemiş terminal bronşiollerin kesitinin yuvarlak kistik boşluk gibi görünmesiyle yanıltıcı olabilir; bu nedenle ardışık ince kesitlerde dallanma sürekliliği izlenmelidir.
Posterior bazal opasiteyi fibrozis diye adlandırmadan önce pozisyon kaynaklı atelektaziyi dışla; gerekirse yüzüstü seriyle karşılaştır.
UIP paterni ile IPF aynı kavram değildir; ilaç, bağ dokusu hastalığı ve maruziyet öyküsü etiyolojik yorumu değiştirebilir.
