Özet
Özofagus adenokarsinomu Barrett epitel zemininde gelişebilir; gastroözofageal bileşke tümörlerinde anatomik merkezin yeri Siewert sınıflamasını etkiler. Kontrastlı BT, özofagus tümörünün duvar ve lümen bulgularını, komşu yapılarla yağ planlarını, lenf düğümlerini ve uzak metastazları değerlendirmede kullanılır. BT, T1 ile T2 özofagus tümörlerini ayıramaz; tanı endoskopik değerlendirme ve biyopsiyle, duvar katmanlarının lokal evrelemesi ise gerektiğinde endoskopik ultrasonla yapılır. BT'nin erken evre hastalıktaki duyarlılığı düşük olduğundan primer tümörün veya karaciğer ve diğer uzak metastazların saptanmaması evrelemenin eksik kalmasına yol açabilir.
Okuma sırası
- distal torasik özofagus. Diyafram hiatusunun hemen üstünden başlayıp kardiyaya in; duvarın tek bir noktada eksantrik kabardığı ve lümenin düzensiz daraldığı kesitleri bul.
- GE bileşke ve mide kardiyası. Koronal veya sagittal rekonstrüksiyonda kitlenin en kalın bölümünü GE bileşkeye göre konumlandır; Siewert tarifinde esas alınan merkez ile uzanımı birbirinden ayır.

- Portal tanısal
- Tek fazlı intravenöz kontrastlı toraks-abdomen BT'de distal özofagus ve GE bileşkede fokal/asimetrik mural yumuşak doku, lümen darlığı, çevre yağ planları, bölgesel nodlar ve karaciğer odakları birlikte incelenir; yeterli özofagus distansiyonu çökmeye bağlı yalancı kalınlaşmayı azaltır.
Önerilen pencereler: Mediasten (G 350 / M 50), Batın (G 400 / M 50), Karaciğer (G 150 / M 30), Yumuşak doku (G 400 / M 40).
BT bulguları
- Distal torasik veya abdominal özofagusta eksantrik ya da çevresel, düzensiz mural yumuşak doku; uzunlamasına devamlılığını bileşkeye kadar izle.
- Tümör merkezinin anatomik GE bileşkenin üstünde, bileşke düzeyinde veya mide kardiyası tarafında kalması; büyük kitlede merkez ve uzanım ayrı tarif edilir.
- İnfiltratif özofagus karsinomu düzensiz darlığa yol açabilir.
- Tümör dış sınırında silinme veya periezofageal yağda çizgilenme; bu görünüm komşu organ invazyonunu tek başına kanıtlamaz.
- Periezofageal yağ infiltrasyonu veya tümör ile komşu mediastinal yapılar arasındaki yağ planının kaybı lokal yayılım şüphesi uyandırır; bu bulgular tek başına invazyonu kanıtlamaz.
- Sol gastrik damar boyunca, gastrohepatik ligamentte, çölyak çevresinde veya alt mediastende yuvarlaklaşmış, nekrotik ya da kümelenmiş düğümler.
- Karaciğerde çoklu hipodens odaklar; akciğer, adrenal bez, kemik veya peritonda eşlik eden uzak hastalık bulguları.
- Kontrastlı BT, özofagus kanserinde lokal yayılımı ve uzak metastazları değerlendirmek için kullanılır; erken evre hastalıkta duyarlılığı düşüktür ve T1 ile T2 hastalığı ayıramaz.

Ölçütler ve sınıflamalar
- Siewert GE bileşke adenokarsinomu sınıflaması
- Tip I: tümör merkezi GE bileşkenin 1–5 cm proksimalinde. Tip II: merkez bileşkenin 1 cm üstü ile 2 cm altı arasında. Tip III: merkez bileşkenin 2–5 cm distalinde ve tümör bileşkeye aşağıdan ilerler. Bu anatomik sınıflama tümör merkezini tarif eder; BT’de büyük tümörlerde endoskopik ve klinik korelasyon gerekebilir. Siewert tipi ile TNM evrelemesi ayrı kavramlardır: AJCC 8. baskıda EGJ’yi tutan ve tümör merkezi kardiyanın proksimal midesine en çok 2 cm uzanan tümörler özofagus şemasında, 2 cm’den fazla uzananlar mide şemasında evrelenir.
Normalde
Normal GE bileşkede distal özofagus, diyafram hiatusundan geçip mide kardiyasına düzgün biçimde devam eder; yeterince açılmış lümenin çevresindeki duvar ince ve simetriktir. Kıyaslamada aynı düzeydeki ön ve arka duvar kalınlığını, dış konturun düzgünlüğünü, kardiya kıvrımlarına geçişi ve sol gastrik damar çevresindeki yağ aralığını karşılaştır.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

- Çökmüş veya peristaltik özofagus duvarı yalancı kalın görünebilir; aynı anormalliğin ardışık kesitlerde sürdüğünü ve dış konturu bozduğunu doğrula.
