Özet

Özofagusta takılı kalan kemik, metal parça veya protez gibi cisimler lümeni tıkayabilir; sivri uçlu cisimler duvarı delip boyun ya da mediastene ilerleyebilir. BT, özellikle grafide görünmeyen cismi, cismin duvarla ilişkisini ve perforasyonun çevresel etkilerini aynı incelemede gösterir. İntravenöz kontrastlı BT veya uygun durumda BT özofagografi, şüpheli perforasyon ve yabancı cisim komplikasyonlarının yayılımını ortaya koymada önemlidir. Migrasyon ya da sızıntının gözden kaçması mediastinit, apse, plevral komplikasyon ve büyük damar komşuluğunda yaşamı tehdit eden yaralanmanın tanısını geciktirebilir.

Faz ve pencere

Kontrastsız tanısal
Kemik, metal veya yoğun protez parçası özofagus lümeninde ya da duvar dışında yüksek atenüasyonlu çizgisel/noktasal yapı olarak seçilebilir; çevre yumuşak dokuların inflamasyonu ve damar ilişkisi sınırlı değerlendirilir.
Venöz tanısal
İntravenöz kontrastlı göğüs-boyun BT'de özofagus duvar kalınlaşması ve çevre yağ dokusu kirlenmesi, ekstraluminal gaz, sıvı/koleksiyon ve plevral sıvı araştırılır; BT özofagografide lümen dışına kontrast geçişi perforasyon lehinedir.

Önerilen pencereler: Mediasten (G 350 / M 50), Yumuşak doku (G 400 / M 40), Akciğer (G 1500 / M -600).

BT bulguları

  • İntraluminal yabancı cisim — özofagus lümeni içinde keskin kenarlı kemik, metal parça veya protez; çok düzlemli rekonstrüksiyonda uzun ekseni ve impaksiyon yeri izlenir.
  • Özofagus duvarını geçen cisim — Cismin bir ucu lümen içinde, devamı özofagus duvarının dışında izlenir; komşu anatomik yapılarla ilişkisi değerlendirilir.
  • Perforasyon odağında fokal duvar kalınlaşması veya duvar süreksizliği — cismin temas ettiği segmentte çevresel yumuşak doku reaksiyonu eşlik edebilir.
  • Paraözofageal ekstraluminal gaz — özofagus konturu dışındaki küçük hava kabarcıkları veya mediastinal serbest hava perforasyonu destekler.
  • Paraözofageal yağ dokusunda çizgilenme — Özofagus perforasyonuna komşu yağ dokusunda çizgilenme görülebilir.
  • Mediastinal sıvı ya da koleksiyon — cismin komşuluğunda sıvı yoğunluklu alan, organize olguda çevresel kontrastlanma gösterebilir.
  • Plevral eşlik — özellikle alt özofagus yaralanmasında plevral efüzyon, hidropnömotoraks veya komşu akciğerde kompresif değişiklik görülebilir.
  • Cismin aort ve diğer büyük damarlarla ilişkisini değerlendirin; komşu damar yaralanması veya aortözofageal fistül şüphesini kontrastlı BT’de araştırın.
  • Yabancı cismin uzun eksenini ve uçlarının aort, arkus dalları veya diğer büyük damarlarla ilişkisini çok düzlemli görüntülerde değerlendirin; kontrastlı BT ile eşlik eden damar yaralanması bulgularını araştırın.

Normalde

Şüpheli seviyeyi komşu kraniyal ve kaudal özofagus segmentleriyle karşılaştırın: lümen içi cismi, duvar konturundaki kesintiyi ve normal paraözofageal yağ planının kaybını ayrı ayrı arayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Mediastinal hava
trakeobronşiyal ağaç veya akciğer kaynaklı olabilir; özofagus duvar kusuru ve cisme komşu fokal inflamasyon bulunmayabilir.
Özofagus duvar kalınlaşması neoplastik kaynaklı olabilir; yabancı cisim ise BT'de lineer hiperdens yapı olarak görülebilir.

Tuzaklar

  • Balık kılçığı veya ince kemik kontrastsız kesitte özofagus duvarına paralel uzanıp kısmi hacim etkisiyle kaybolabilir; aksiyel incelemeye ek olarak koronal ve sagittal rekonstrüksiyonları takip edin.
  • Metal cisim çevresindeki çizgilenme artefaktı duvarı ve komşu damar sınırını örtebilir; cismin iki ucunu farklı düzlemlerde lokalize edip çevre yumuşak doku penceresinde yeniden inceleyin.
  • Cisim artık lümende görünmüyorsa perforasyon dışlanmaz; migrasyon hattı boyunca paraözofageal gaz, inflamasyon veya koleksiyonu ve boyun/mediasten uzanımını araştırın.
  • İnceleme alanı yalnız göğüsle sınırlı kalırsa servikal özofagus cismi ya da yukarı uzanan enfeksiyon atlanabilir; klinik şüpheye göre orofarinks ve torasik inlet de kapsanmalıdır.
  • Balık kılçığı grafide görünmeyebilir; klinik şüphe sürüyorsa ince kesitli BT ve çok düzlemli rekonstrüksiyonlar yabancı cismin yerini ve komplikasyonlarını göstermeye yardımcı olur.
