Schwannom ve nörofibrom BT bulguları

Özet

Schwannom ve nörofibrom, özellikle erişkinlerde arka mediastenin paravertebral bölümünde görülen periferik sinir kılıfı tümörleridir. BT, kitlenin sinir kökü ve nöral foramenle ilişkisini, iç yapısını ve vertebra-kosta üzerindeki bası etkisini gösterir; kanal içi uzanım şüphesinde MR anatomiyi daha iyi tamamlar. Düzgün sınırlı kitle çoğunlukla yavaş büyümeyi düşündürürken, belirgin invazyon ve kemik yıkımı atipik davranış açısından önemlidir. Foraminal veya spinal kanal uzanımını atlamak sinir yapılarının bası ve cerrahi planlamasını eksik bırakabilir.

Faz ve pencere

Kontrastsız tanısal: Paravertebral kitlenin yumuşak doku dansitesi, yağlı veya kistik alanları, kalsifikasyonu ve vertebra-kosta konturu gösterilir; lezyonun sinir kökü doğrultusundaki yerleşimi seçilir. Bu seri yerleşim, temel atenüasyon ve kemik remodelingi için yararlıdır; solid komponentin kontrastlanmasını göstermez ve tek başına lezyon karakterizasyonunu tamamlamaz.
Kontrastlı: Kontrastlı toraks BT varsa solid bileşenin değişken kontrastlanması, kistik/nekrotik alanların düşük atenüasyonu ve büyük kitlelerde heterojenite aranır; belirgin infiltratif kenar benign sinir kılıfı tümörü varsayımını zayıflatır.
Kemik algoritması tanısal: Nöral foramen genişlemesi, vertebra gövdesinde bası erozyonu, kostada çentiklenme ve varsa kanal yönünde uzanan dumbbell biçimli kitlenin kemik sınırları değerlendirilir.

Önerilen pencereler: Mediasten (G 350 / M 50), Kemik (G 1800 / M 400).

BT bulguları

Paravertebral yerleşim — kitle vertebra yanındaki posterior mediastinal yağ planından gelişir ve sinir kökü hattına oturur.
Düzgün sınırlı yuvarlak veya oval kontur — schwannom ve nörofibrom çoğu kez iyi sınırlı yumuşak doku kitlesi görünümündedir.
Nöral foramen genişlemesi — foramenden geçen kitle eşlik ediyorsa kanal dışı ve kanal içi bileşenler kum saati/dumbbell şekli oluşturabilir.
Dejeneratif heterojenite — schwannomda kistik değişiklik, kanama veya nekroz kitlenin atenüasyonunu ve kontrastlanmasını yamalı hale getirebilir.
Basınç remodelingi — uzun süreli büyümede vertebra veya komşu kostada düzgün kortikal oyulma görülebilir; agresif kemik destrüksiyonundan ayrılmalıdır.
Nörofibromda sinir boyunca uzanım — kitle sinir hattını izleyebilir; çok odaklı veya pleksiform görünüm NF1 bağlamını düşündürür.
Komşu yapı etkisi — büyük kitle özofagus, hava yolu veya plevrayı iter; damar içine uzanım tipik değildir.
MR, nöral yapıları değerlendirmede BT'den üstündür; BT ise kemik remodelingi gibi bulguları göstererek tamamlayıcı bilgi sağlar.

Normalde

Dumbbell tümör, nöral foramen üzerinden intra- ve ekstraspinal uzanım gösterir; foraminal uzanımı değerlendirin. Kitle varsa BT veya MR'da nöral foramenden geçerek kanal içine uzanımını ve komşu yapılarla ilişkisini değerlendirin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Ganglion hücreli tümör: daha uzun kraniokaudal uzanım ve sempatik zincir boyunca yerleşim periferik sinir kılıfı kitlesinden ayrılır.
Paragangliom: yoğun arteriyel kontrastlanma ve belirgin besleyici damarlar kılıf tümöründen daha vasküler görünür.
Ekstramedüller hematopoez: çoğunlukla bilateral düzgün paravertebral kitleler ve kronik hematolojik hastalık bağlamı beklenir.
Meningosel: BOS dansitesinde kese, genişlemiş foramen ve spinal kanal ile devamlılık gösterir.
Lenfoma veya metastaz: çok odaklı adenopati, infiltratif yumuşak doku ya da eşlik eden akciğer/kemik lezyonları eşlik edebilir.

Tuzaklar

Tek kesitte foramen genişliği normal varyant izlenimi verebilir; aynı tarafta kitleyle devamlılığı ve karşı taraf simetrisini çok düzlemli inceleyin.
Schwannomun kistik dejenerasyonu kitleyi basit kist gibi gösterebilir; duvar ve solid nodüler bileşenin kontrastlanmasını kontrol edin.
Düzgün kemik basısı malign invazyon anlamına gelmez; korteksin keskin remodelingi ile düzensiz yıkımı ayırın.
Schwannom ve nörofibromun görüntüleme bulguları örtüşebilir; kesin tanı gerektiğinde histopatolojik değerlendirme gerekir.
