Özet
Perine ağrısı ve cilt kızarıklığında yüzeyel selülit, girişim sonrası hava ve Fournier gangreni görüntüde birbirine benzeyebilir. BT, gazın cilt altı dokuda mı derin fasyada mı bulunduğunu, lümene ait olup olmadığını ve apse ile derin yayılımı göstererek enfeksiyonun kapsamını açıklar; anal fistül yolunun tamamı BT’de güvenilir biçimde seçilemeyebilir. Nekrotizan enfeksiyonun erken evrelerde gaz üretmemesi nedeniyle yanlış negatif yorumlama riskini azaltmak için radyologlar derin fasyal sıvı ve kontrastlanmama bulgularını önceliklendirmelidir.

- BT'de gaz oluşumu ve fasyal planlara yayılım değerlendirilir; derin veya komplike enfeksiyonda kontrastlı BT, fasyal tutulumu, yayılımı ve apseyi değerlendirmek için temel görüntüleme yöntemidir.
- Portal tanısal
- Cilt ve yüzeyel yağ dokusu ödemine ek olarak çevresel kontrastlanan apse, derin fasya sıvısı ve yayılımı değerlendirilir. Anal fistül yolu BT’de belirsiz veya eksik izlenebilir; fistül haritalaması gerekiyorsa MR, uygun durumlarda endoanal US daha elverişlidir.
Önerilen pencereler: Yumuşak doku (G 400 / M 40), Batın (G 400 / M 50).

- Selülitte cilt kalınlaşması ve yüzeyel yağ dokuda retiküler dansite artışı; derin fasyada gaz olmaması
- Fournier lehine fasyal yüzey boyunca dizilen gaz kabarcıkları ve asimetrik derin fasya kalınlaşması
- İşlem/kateter giriş yoluyla aynı doğrultuda sınırlı cilt altı hava; komşu derin planların korunması
- Fasyaya komşu, merkezinde düşük dansiteli sıvı ve çevresinde kontrastlanma bulunan apse
- Perianal koleksiyon ile anal kanal veya rektum arasında devamlı fistül yolu
Normalde
Normal perineal cilt ince, iki yanlı benzer kalınlıkta; subkutan yağ ve derin fasyal planlar temiz, gazsız görünür. Şüpheli tarafta değişikliğin yalnız cilt/yağ dokusuyla mı sınırlı kaldığını, yoksa yüzeyel fasyayı geçip derin kompartımana uzandığını karşılaştır.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
