Testis tümöründe kemik/visseral metastaz

Özet

Testis germ hücreli tümöründe akciğer hematojen uzak yayılımın tipik alanlarından biridir; karaciğer, kemik ve diğer organ metastazları daha ileri hastalıkta görülebilir. Kontrastlı toraks-abdomen-pelvis BT, akciğer nodüllerini ve visseral odakları saptar; kemik pencereleri litik lezyonları ve kortikal yıkımı gösterir. Testis germ hücreli tümörlerinin sistemik evrelemesinde kontrastlı toraks-abdomen-pelvis BT temel yöntemdir; PET-BT ve kemik sintigrafisi tanısal belirsizlikler nedeniyle rutin kullanılmaz. Kemik metastazları başlangıçta asemptomatik olabileceğinden, BT'de kemik bütünlüğü ve MR'da epidural ya da diğer yumuşak doku uzanımı değerlendirilmezse lezyon yayılımı eksik kalabilir.

Portal tanısal

Kontrastlı toraks-abdomen-pelvis BT'de yuvarlak solid pulmoner nodüller/kitleler, karaciğerde hipodens veya heterojen kontrastlanan metastazlar ve kemikte litik destrüksiyon aranır; kemik bulgularında kemik algoritmasıyla korteks ve yumuşak doku bileşeni ayrıca incelenir.

Önerilen pencereler: Akciğer (G 1500 / M -600), Mediasten (G 350 / M 50), Karaciğer (G 150 / M 30), Kemik (G 1800 / M 400), Batın (G 400 / M 50).

BT bulguları

• Pulmoner metastazlar tek veya çok sayıda solid nodül, daha büyük kitle ya da yaygın mikronodüler dağılım şeklinde olabilir.
• Hiler ve mediastinal lenf nodları akciğer parankim bulgularına eşlik ederek supradiyafragmatik yayılımı gösterebilir.
• Karaciğer metastazları portal fazda normal parankime göre düşük atenüasyonlu, bazen heterojen kontrastlanan odaklar olarak belirir.
• Litik kemik metastazında medüller kemik yoğunluğu azalır; ilerlemiş odakta kortikal incelme veya yıkım gelişebilir.
• Vertebra ya da pelvis lezyonundan çıkan ekstraosseöz yumuşak doku bileşeni epidural veya komşu organlara uzanımı düşündürür.
• Yaygın testis germ hücreli tümöründe karaciğer ve retroperitoneal metastazların yanı sıra renal hilus lenfadenopatileri de BT'de görülebilir.
• Koryokarsinom bileşenli hastalıkta hematojen yayılım ve kanamalı uzak metastazlar görülebileceğinden şüpheli odaklar klinik ve MR ile gerektiğinde tamamlanır.

UICC/AJCC TNM evreleme ve IGCCCG prognoz sınıflaması

Testis germ hücreli tümörlerinin sistemik yayılımı IGCCCG prognoz sınıflamasına göre değerlendirilir; akciğer metastazı ile akciğer dışı visseral metastazların prognoz değeri farklıdır.

Normalde

Testis germ hücreli tümöründe BT, uzak metastaz alanlarının saptanması için evrelemede kullanılır; şüpheli akciğer, karaciğer veya kemik odakları bu kapsamda değerlendirilir. Şüpheli nodülü çevre damarlarla, karaciğer odağını komşu sağlam parankimle, kemik odağını karşı taraf aynı vertebra veya pelvis kemiğiyle kıyaslayın; yumuşak doku penceresinde eşlik eden ekstraosseöz uzanımı arayın.

Ayırıcı tanı

Enfeksiyöz pulmoner nodüller

eşlik eden konsolidasyon, kavite veya bronşiyoler dağılım ve klinik enfeksiyon öyküsü metastazdan ayrılmaya yardım eder.

Karaciğer kisti

suya yakın homojen içerik ve kontrastlanmayan ince duvar gösterir; solid metastaz gibi iç kontrastlanma yapmaz.

Atipik hemanjiyom

kontrastlı serilerde çevresel nodüler dolum ve gecikmiş dolum örüntüsü gösterebilir.

Kemik adası

yoğun, düzgün sınırlı sklerotik odaktır; litik kortikal destrüksiyon ve yumuşak doku kitlesi oluşturmaz.

Myelom veya başka metastatik malignite

her ikisi de çok odaklı hastalık oluşturabilir; bu nedenle yalnızca çok odaklı dağılım ayırıcı tanıyı belirlemez.

Tuzaklar

• Soliter küçük akciğer nodülü tek başına metastaz kanıtı değildir; önceki görüntülerle büyümeyi ve aynı incelemedeki diğer yayılım alanlarını karşılaştırın.
• Karaciğerin kontrastsız veya uygunsuz zamanlanmış incelemesinde küçük metastazlar belirsiz kalabilir; şüpheli odağı portal faz ve gerekirse MR ile netleştirin.
• Litik vertebra odağında kemik algoritması kortikal kaybı gösterir, ancak epidural yumuşak dokuyu dışlamaz; yumuşak doku penceresini de kontrol edin.
• Non-seminomatı germ hücreli tümörlerde kemik metastazı nadir olmakla birlikte omurgayı sık tutabilir ve çoğunlukla asemptomattir; rezidü kitlelerin histolojisi görüntüleme ile kesin ayırt edilemez.
