Özet

Ağrılı, şiş veya hareketi kısıtlı dirsekte eklem enfeksiyonu kıkırdak ve komşu kemiği geri dönüşsüz biçimde hasarlayabileceği için acil tanınmalıdır. BT, eklem sıvısını ve çevre yumuşak dokudaki yayılımı gösterir; ince kemik kesitleri kortikal erozyon, destrüksiyon ve periostal değişiklikleri belirginleştirir. Erken septik artritte BT normal olabilir veya yalnızca özgül olmayan yumuşak doku şişliği ve eklem efüzyonu gösterebilir; efüzyon enfeksiyonu kanıtlamaz. Tanının gecikmesi kalıcı eklem hasarı ve işlev kaybına yol açabilir.

Okuma sırası

  1. Humeroulnar eklem ve olekranon fossası. Aksiyel kesitlerde olekranon fossasına uzanan posterior eklem resesini takip et; kapsülün sıvıyla genişleyip genişlemediğini karşı dirsek eklem konturuyla kıyasla.
  2. Olekranon fossası çevresi ile triseps ve brakialis kas komşuluğu. Olekranon fossasının çevresindeki yağ dokusunu ve triseps ile brakialis kaslarını çevreleyen planları izle; inflamatuvar çizgilenmenin eklem dışına hangi kompartımana yayıldığını belirle.
  3. Periartiküler yumuşak doku koleksiyonu. Kontrastlı kesitlerde eklem resesinden ayrı duran sıvı odağını bul; merkez atenüasyonunu çevre duvarın kontrastlanmasıyla birlikte değerlendir.
  4. Dirsek eklem yüzleri ve komşu korteks. Distal humerus kondillerinin, radius başının ve olekranonun eklemle temas eden korteksini eklem dışındaki sağlam korteksle karşılaştır; marjinal kemik kaybı veya kesintili periostal yeni kemik ara.

BT bulguları

  • Olekranon fossası, koronoid fossa ve ön eklem reseslerinde eklem kapsülünü genişleten sıvı atenüasyonlu efüzyon.
  • Eklem kapsülü çevresindeki yağ planlarında silinme ve kas kompartımanlarına uzanan yumuşak doku ödemi; enfeksiyon için tek başına özgül değildir.
  • Eklem çevresindeki yumuşak doku koleksiyonlarının varlığını ve varsa çevresel kontrastlanmayı değerlendir.
  • Distal humerus kondilleri, radius başı ve olekranonun eklem yüzüne komşu korteksinde marjinal erozyon, kortikal düzensizlik veya kemik kaybı.
  • Kemik enfeksiyonunda BT kortikal ve trabeküler kemik yıkımını gösterebilir.
  • Yumuşak doku ya da eklem içindeki gaz, eklem enfeksiyonuna yol açabilen penetran travma veya inokülasyon öyküsüyle birlikte değerlendirilmelidir.

Normalde

Normal dirsek BT'sinde humeroulnar ve radiokapitellar eklem aralıkları belirgin sıvıyla genişlemez; olekranon ve koronoid fossaların çevresindeki yağ planları seçilebilir. Kemik enfeksiyonunda BT, kortikal ve trabeküler kemik yıkımını gösterebilir.

Tuzaklar

  • BT'de efüzyonun yoğunluğu steril sıvıyı püyden güvenilir biçimde ayırmaz; görüntüleme eklem aspirasyonunun yerine geçmez.
  • Hastalığın ilk haftasında BT büyük ölçüde normal olabilir; BT'de kemik yıkımının görülmemesi kemik iliği tutulumunu dışlamaz, çünkü MR kemik iliği alanını daha iyi gösterir. Erken medüller tutulumda MR, kemik iliği değişikliklerini değerlendirmek için daha duyarlıdır.
