Özet

El bileği travması veya distal radius kırığı sonrasında ulnar stiloid ve distal ulna eklem yüzü birlikte incelenmelidir. İnce kesit kontrastsız BT, stiloid kırık düzeyini, sigmoid çentiğe uzanımı ve DRUJ kemik uyumunu göstermede yararlıdır. Ulnar stiloid taban veya uç kırığının yeri tek başına TFCC foveal bütünlüğünü ya da DRUJ stabilitesini göstermez; kemik BT bulgusu yumuşak doku bütünlüğünü tek başına değerlendirmez. DRUJ yaralanmaları önkol pronasyon-supinasyonunda ağrı veya hareket kısıtlılığına yol açabileceğinden eklem uyumu ayrıca değerlendirilir.

Faz ve pencere

Kontrastsız tanısal
Kontrastsız BT'de distal ulna ve ulnar stiloid kırıkları ile distal radius kırığının sigmoid çentiğe uzanımı, deplasmanı ve eklem içi basamağı değerlendirilir. BT, distal radius kırığının sigmoid çentiğe uzanımını ve distal radioulnar eklem tutulumunu değerlendirmede yararlıdır.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Ulnar stiloid uç kırığında küçük distal fragman görülebilir; uç ya da taban seviyesi tek başına TFCC foveal yırtığını ve DRUJ stabilitesini belirlemez.
  • Ulnar stiloid taban kırığı stiloid kökü düzeyindedir; yüzeyel radioulnar ligament lifleri tabana, derin lifler ulnar foveaya tutunur.
  • Distal ulna başı kırığında eklem yüzüne uzanım, basamak veya parçalanma ulnar başın artiküler konturunu bozabilir.
  • Sigmoid çentik kenar kırığında fragman eklem yüzüne uzanabilir ve DRUJ kemik uyumunu bozabilir.
  • DRUJ subluksasyonunda ulnar başın sigmoid çentikle hizası bozulur; görünüm pronasyon-supinasyonla değiştiğinden inceleme pozisyonuyla birlikte değerlendirilmelidir.
  • Eşlik eden distal radius kırığı sigmoid çentiğe uzanırsa DRUJ kemik uyumunu doğrudan bozabilir; ulnar stiloid bulgusu ayrı tanımlanmalıdır.

Normalde

Normal DRUJ'de sigmoid çentiğin içbükey eklem yüzü daha dışbükey ulna başıyla eklemleşir; sigmoid çentik şekli doğal olarak değişken olabilir. İlişkiyi önkol rotasyon pozisyonuna göre değerlendirin; pozisyon değişimi ulnar başın normal konumunu etkiler.

Ulnar stiloid kırığında komşu TFCC ve eklem kapsülü lezyonları da göz önünde bulundurulmalıdır.
Kronik ulnar stiloid kaynamaması
psödoartroz veya stiloid kökü yakınında yeni eklemleşme görülebilir; önceki görüntülerle birlikte değerlendirilir.
Distal radius sigmoid çentik kırığı
primer kırık çizgisi radiusun ulnar eklem yüzündedir; eşlik eden ulnar stiloid fragmanı ayrıca tanımlanmalıdır.
Pozisyona bağlı DRUJ asimetrisi
pronasyon-supinasyon eklem ilişkisini değiştirir; subluksasyon yorumundan önce edinim pozisyonunu göz önünde bulundurun.

Tuzaklar

  • Stiloid uç kırığını taban kırığıyla karıştırmayın; yüzeyel radioulnar ligament lifleri tabana, derin lifler foveaya tutunur ve kırık seviyesi tek başına TFCC durumunu göstermez.
  • Ulna başının sigmoid çentiğe göre rotasyonel yönelimi değişken olabilir; tek bir aksiyel kesitteki görünümü tek başına subluksasyon kabul etmeyin.
  • Stiloid kırığı görülünce DRUJ'yi normal varsaymayın; sigmoid çentik, ulnar baş ve distal radius eklem yüzünü ayrı ayrı değerlendirin.
