Anevrizmal kemik kisti (AKK), çoğunlukla çocuk ve genç erişkinlerde görülen, kanla dolu boşluklar içeren ekspansil ve lokal agresif kemik lezyonudur. Femur metafizi tipik yerleşimlerden biridir; kortikal incelme varsa patolojik kırık riski değerlendirilmelidir. BT litik lezyonun kemik içi sınırını, kortikal kabuğu, mineralize matriksi ve kırığı gösterir; sıvı-sıvı seviyelerinin ve yumuşak doku uzanımının karakterizasyonunda MR daha duyarlıdır. Sıvı-sıvı seviyeleri AKK’ye özgü değildir; telanjiektatik osteosarkom gibi malign taklitçilerin atlanmaması için solid bileşen ve kemik destrüksiyonu dikkatle aranmalıdır.
Okuma sırası
- Femur metafizi. Distal femur metafizini koronal kemik penceresinde tara; medüller kaviteye göre eksantrik yerleşen, kemiği genişleten litik odağı sınırla.
- Lezyon çevresi korteksi. Lezyonun en geniş olduğu düzeyde kortikal kabuğu baştan sona izle; incelme, kesinti ve karşı kortekse göre genişleme derecesini göster.
- Lezyon içi. Aksiyel kesitlere geçerek kavite içindeki ince septaları, sıvı-sıvı seviyelerini ve mineralize odakları ayırt et; sıvı-sıvı seviyesi varsa tanıya özgü olmadığını belirt.
- Korteks ve komşu yumuşak doku. Son olarak kortikal kırık hattını ve lezyon dışına uzanan solid bileşeni ara; kırık, kortikal yıkım veya agresif yumuşak doku uzanımı varsa lezyonu yalnızca benign AKK olarak yorumlama.

BT bulguları
- Metafiz merkezli, çoğunlukla eksantrik ve ekspansil osteolitik lezyon femur boyunca korteksi inceltir.
- Lezyon içindeki ince septalar çok odacıklı bir görünüm oluşturabilir.
- Çevresel ince reaktif kemik kenarı ve iyi tanımlı geçiş zonu benign davranış lehine olsa da kesin tanı koydurmaz.
- Kortikal kabuk belirgin incelmiş veya kesintiye uğramış olabilir; patolojik kırık eşlik edebilir.
- BT’de hemorajik boşluklar arasında sıvı-sıvı seviyeleri seçilebilir; bu görünüm AKK için özgül değildir.

Ölçütler ve sınıflamalar
- Enneking benign kemik lezyonu aktivite evrelemesi
- Evre 1 (latent/inaktif): sınırlı, ekspansil olmayan lezyon; sklerotik kenar ve korunmuş korteks. Evre 2 (aktif): genişleme ve kortikal incelme gelişir; çevresel reaktif kemik görülebilir. Evre 3 (agresif): hızlı ekspansiyon, kemik destrüksiyonu ve çevre yumuşak dokuya uzanım olabilir. Bu aktivite evrelemesi, Capanna’nın I–V morfolojik alt tiplerinden farklıdır.
Normalde
Aynı seviyedeki normal femur metafizinde kortikal kabuk kesintisiz, medüller trabeküller düzenli ve ekspansil litik boşluk bulunmaz. Karşı femurun aynı bölgesiyle kıyaslayarak korteks kalınlığını, kemik çapındaki genişlemeyi ve lezyonun medüller eksene göre eksantrik konumunu göster.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
