Özet

Femur metastazı kemiğin yapısal dayanımını azaltarak patolojik kırığa yol açabilir. BT, femoral metastazlarda ek lezyonları saptamaya ve kortikal tutulumun yayılımını daha doğru değerlendirmeye yardımcı olabilir. BT, femoral metastazlarda ek lezyonların saptanmasına ve kortikal tutulumun daha doğru değerlendirilmesine yardımcı olabilir. Kırık hattı altta yatan odaktan geçiyorsa kemik iliği yayılımı ve varsa yumuşak doku uzanımı ayrıca değerlendirilmelidir.

Kontrastsız tanısal
BT’de femurdaki litik lezyonlar, kortikal kesinti, kırık ve varsa yumuşak doku uzanımı değerlendirilebilir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Mirels değerlendirmesinde lezyon tipi litik, mikst veya blastik olarak sınıflandırılır; femur metastazında patolojik kırık gelişebilir.
  • Litik odakta endosteal oyulma veya kortikal tutulum BT’de izlenebilir; kortikal tutulum uzun kemik kırık riski değerlendirmesinde kullanılan bir bulgudur.
  • BT, femoral metastazlarda kortikal tutulumun ve ek kemik lezyonlarının daha doğru değerlendirilmesine yardımcı olabilir.
  • Kırıkla ilişkili agresif kemik lezyonunda tümörün normal kemik iliğini değiştirmesi ve komşu yumuşak doku komponenti görülebilir.
  • Özellikle prostat kaynaklı bazı osteoblastik metastazlar sklerotik görünse de tümörle ilişkili kemik dokusu kırılgan olabilir.
  • Birden çok kemik odağı saptanırsa değerlendirme tek lezyonla sınırlanmamalıdır; Bone-RADS algoritmaları yalnız tekil lezyonlar için tanımlanmıştır.

Mirels skoru
Mirels skoru, uzun kemik metastazlarında anatomik yerleşim, ağrı, lezyon tipi ve boyutunu kullanarak patolojik kırık riskini değerlendiren bir araçtır.

Normalde

Normal femurda korteks kesintisizdir ve medüller kemikte fokal litik, sklerotik veya mikst lezyon bulunmaz. Kırık hattını, kortikal tutulum veya yumuşak doku uzanımıyla birlikte değerlendir; bu bulgular kemik lezyonlarında ileri değerlendirme gerektirebilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Osteoporotik kırık
fokal lezyonda kortikal tutulum veya yumuşak doku uzanımı tümör açısından kuşku doğurur; bu bulgular metastaza özgü değildir.
Osteomalaziye bağlı yetersizlik kırığında femurda psödo-kırıklar görülebilir.
Paget ilişkili kırık
Paget hastalığında proksimal femurda hafif eğrilik ve intertrokanterik kırık birlikte görülebilir.
Agresif primer kemik tümörü
permeatif yıkım ve komşu yumuşak doku komponenti agresif lezyonu düşündürür; bu bulgular tek başına metastazı kanıtlamaz.

Tuzaklar

  • Özellikle prostat kaynaklı osteoblastik metastazları yalnız yoğun sklerotik görünüme dayanarak sağlam kabul etme; bu lezyonlardaki anormal kemik dokusu kırılgan olabilir.
  • Kanser öyküsü olan hastada yeni kemik bulguları metastaz dışında yetersizlik kırığı gibi nedenlerle de ilişkili olabilir.
  • BT’de kortikal tutulumun yayılımını değerlendir; BT, femoral lezyonlarda kortikal tutulumun daha doğru değerlendirilmesini sağlayabilir.
