Özet

SLIC, subaksiyel servikal travmayı morfoloji, diskoligamentöz kompleks (DLC) ve hastanın nörolojik durumunu bir araya getirerek tanımlayan bir sınıflama ve şiddet puanıdır. BT; kompresyon, burst, distraksiyon ve translasyon/rotasyon paternini, faset ilişkisini ve kemik hizalanmasını gösterir; DLC'nin kendisi ve nörolojik muayene BT'den kesinleştirilemez. Bu nedenle puanın tüm bileşenlerini yalnız görüntüden üretmek, bağ yaralanmasını veya nörolojik etkilenmeyi yanlış sınıflandırabilir. Yaralanma paterninin standart biçimde aktarılması klinisyenler arası iletişimi kolaylaştırır ve omurga stabilitesinin değerlendirilmesine yardımcı olur.

Kontrastsız tanısal
BT, subaksiyel servikal yaralanmalarda morfolojiyi değerlendirmede kullanılır; SLIC morfoloji bileşeninde kompresyon, burst, distraksiyon ve rotasyon/yer değiştirme paternleri yer alır. İyotlu kontrast SLIC morfolojisi için gerekmez.
Kemik algoritması tanısal
BT subaksiyel servikal yaralanmanın morfolojisini değerlendirir; servikal omurga yaralanmalarında MR, potansiyel olarak instabil yaralanmalar hakkında ek bilgi sağlayabilir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Kompresyon morfolojisi — vertebra gövdesinde yükseklik kaybı veya son plak çökmesi vardır; posterior duvarın tutulumu ayrıca araştırılır.
  • Burst morfolojisi — gövde kırığı posterior duvara uzanır; retropulsiyon spinal kanalı daraltabilir.
  • Distraksiyon morfolojisi — komşu vertebralar arasında disk aralığı veya posterior elemanlar açılır; tek başına kemik kopması DLC yırtığını doğrudan göstermez.
  • Rotasyon/translasyon morfolojisi — vertebra hizası veya yönelimi bozulur; faset ekleminde bindirme, kilitlenme ya da çıkık eşlik edebilir.
  • DLC için dolaylı BT işaretleri — disk aralığı açılması, faset diastazı/perching, interspinöz açılma ve uyumsuz hizalanma şüphe doğurur; bağın doğrudan görüntüsü değildir.
  • Spinal kanal — burst kırığında geriye yer değiştiren kemik parçaları spinal kanala uzanabilir.
  • Nörolojik bileşen — SLIC, yaralanmanın hastanın sinir sistemi üzerindeki etkisini ayrı bir bileşen olarak puanlar; bu bileşenin değerlendirilmesinde yapılandırılmış nörolojik değerlendirme önemlidir.

SLIC morfoloji-DLC-nöroloji puanlaması
Toplam puan üç bileşenin toplamıdır. Morfoloji 0 normal, 1 kompresyon, 2 burst, 3 distraksiyon, 4 rotasyon/translasyon; DLC 0 sağlam, 1 belirsiz, 2 bozulmuş; nörolojik durum 0 sağlam, 1 kök, 2 tam kord, 3 inkomplet kord olarak puanlanır. Nörolojik defisit varken devam eden kord basısına +1 eklenir. Nörolojik alan klinik muayene gerektirir; literatürde SLICS için gözlemciler arası uyumun tutarsız olduğu bildirilmiştir.

Normalde

Normal C3-C7 BT'de vertebra gövdelerinin ön ve arka korteksleri kesintisiz, posterior vertebral çizgi düzgün, disk aralıkları benzer yükseklikte ve fasetler karşılıklı uyumludur. Yaralanma düzeyini komşu segmentlerle kıyaslayarak korpus yüksekliği, posterior duvar, segmental eksen, faset örtüşmesi ve disk aralığı genişliğine bakın; bu yapısal kıyas nörolojik muayenenin yerini tutmaz.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Dejeneratif spondiloz
osteofit, kronik disk yüksekliği kaybı ve sklerozla seyreder; akut kırık hattı veya yeni segmental açılma beklenmez.

Tuzaklar

  • BT'de disk aralığı açılması veya faset diastazı DLC yaralanmasını düşündürür ama bağ kopmasını tek başına kanıtlamaz; şüpheli olguda MR korelasyonu gerekir.
  • Nörolojik durumu kemik görünümünden türetmeyin; kök, tam kord veya inkomplet kord alanı klinik muayene bilgisidir.
  • Posterior duvarı görmeden vertebra gövdesi çökmesini yalnız kompresyon olarak bırakmayın; retropulsiyon varsa morfoloji burst olabilir.
  • SLIC puanını otomatik tedavi kararı gibi sunmayın; sınıflamanın gözlemciler arası uyumu değişkendir ve klinik bağlamla yorumlanır.
