Akut üst ekstremite iskemisi

Özet

Ani başlayan ağrı, soğukluk, solukluk, parestezi veya nabız kaybı olan hastada üst ekstremite akut iskemisi çoğunlukla kardiyak ya da arteriyel kaynaktan gelen emboliyle gelişir. BTA, tıkanmanın seviyesini ve uzunluğunu, distal yeniden doluşu, kalan açık damarları ve olası emboli kaynağını hızla göstererek girişim planlamasına yardımcı olur. Brakiyal arter sık tutulan emboli yeridir; radial-ulnar dolaşımın da değerlendirilmesi el perfüzyonunu anlamak için gereklidir. Oklüzyonun seviyesi veya distal akım kaybı atlanırsa ekstremite canlılığı ve işlevi tehlikeye girebilir.

Faz ve pencere

BTA tanısal

Arter lümeninde ani segmental kontrast kesilmesi veya dolum defekti, oklüzyonun uzunluğu ve distal kollateral yeniden doluş değerlendirilir; kalp ve büyük damarların kapsanan bölümleri olası emboli kaynağı açısından incelenir.

Gecikmiş tanısal

Arteriyel seride önkola doğru giderek azalan opasifikasyon varsa kısa gecikmeli ek görüntüde radial ve ulnar arterlerin dolması bolus zamanlama hatasını destekler; kalıcı keskin sınırlı kesilme gerçek oklüzyonu destekler.

Önerilen pencereler: Anjiyo (G 600 / M 150), Yumuşak doku (G 400 / M 40).

BT bulguları

• Ani lümen kesilmesi — brakiyal, aksiller veya önkol arterinde keskin sınırlı dolum kaybı ve tıkanmanın proksimal-distal uzanımı kaydedilir.
• Distal yeniden doluş — oklüzyon sonrasında kollaterallerle yeniden opaklaşan radial veya ulnar segment, tıkanıklığın tam uzunluğunu olduğundan kısa gösterebilir.
• Akut emboli çoğunlukla kardiyak kaynaklıdır ve perifer distal arterlerde ya da bifurkasyonlarda ani kan akışı bozukluğuna yol açar; embolik ve trombotik nedenlerin ayrılması yönetimi değiştirir.
• Çoğu akut olay, önceden var olan perifer arter hastalığındaki yerinde tromboz veya daha önce yapılan vasküler girişimler nedeniyle oluşur; bu durum ayırıcı tanıyı etkiler.
• Anevrizma veya psödoanevrizma içi trombüs — aksiller ya da brakiyal kaynaklı kese içindeki trombüsün distal dijital arterlerde embolik defekt oluşturup oluşturmadığı izlenir.

Normalde

Normal BTA’da subklavyen arter lateral birinci kosta kenarında aksillere, teres major alt kenarında brakiyal artere devam eder; antekubital fossada radial ve ulnar dallara ayrılır. Bu dallar el bileğine kadar kesintisiz ve kontrastla doludur; şüpheli kesilme yerini karşı tarafın aynı düzeydeki damarı ve proksimal-distal lümen çapıyla kıyaslayın.

Ayırıcı tanı

Kronik periferik arter hastalığı

uzun segment düzensiz darlıklar, kalsifiye plak ve olgun kollateral ağ akut tek odaklı emboliden ayrılır.

Vazospazm

düzgün veya hafif düzensiz segmental daralma, distal yatakta yaygın ince kalibrasyon; gecikmiş doluşta düzelme görülebilir.

Kontrast bolusunun distal damarları geçmesi

giderek azalan opasifikasyon kısa gecikmiş seride düzelir ve sabit lümen içi pıhtı göstermez.

Travmatik diseksiyon

lümen içinde intimal flep, eksantrik gerçek lümen daralması veya duvar boyunca uzanan hematom eşlik eder.

Eksternal bası

hematom veya ödem damarı dıştan daraltır; lümen içi dolum defekti ve emboli morfolojisi görülmez.

Tuzaklar

• Kapsama boyunca tarayıcının kontrast bolusunu geçmesi radial-ulnar arterleri yalancı tıkalı gösterebilir; giderek silikleşen opasifikasyonda gecikmiş seriyi ve bolus tetikleme yerini kontrol edin.
• Yoğun kontrastlı kol veni veya gövde kaynaklı çizgilenmeler arter lümeninde yalancı dolum defekti yaratabilir; aksiyel kaynak kesitleri ve ortogonal rekonstrüksiyonlarda damarın dışına taşan çizgiyi arayın.
• Tek bir kolda enjeksiyon kaynaklı venöz streak artefaktı aksiller-subklavyen geçişi örtebilir; mümkünse karşı koldan enjeksiyonla elde edilen görüntüyü ve komşu düzlemleri kullanın.
