Özet

Budd–Chiari sendromu, sağ kalp yetmezliği veya konstriktif perikardit olmaksızın, küçük hepatik venüllerden hepatik venlere ve atriyokaval bileşkeye uzanan hepatik venöz çıkışın engellenmesidir; primer intraluminal hastalık veya sekonder dış bası/tümör yayılımı neden olabilir. Kontrastlı BT doğrudan damar lümenini, karaciğer perfüzyonunu, morfolojik yeniden dağılımı ve kollateral yolları birlikte haritalar; Doppler ultrasonografi çoğu olguda ilk incelemedir. Akut yaygın tıkanma hepatik konjesyon ve iskemiye, ağır olguda karaciğer yetmezliğine ilerleyebilir. Doğru zamanlama ve çok fazlı görüntüleme, akut trombüsün erken rekansülizasyonu için kritik olup; kronik olgularda kollateral ağın ve obstrüksiyonun detaylı haritalanması girişimsel planlamanın temelini oluşturur.

Faz ve pencere

Portal tanısal
Hepatik ven veya hepatik VKI'de oklüzyon ya da kompresyon ve kaudat lob hipertrofisi, Budd–Chiari görüntülemesinde değerlendirilen bulgulardır. Portal venöz faz, venöz anatomi ile parankimal perfüzyon farklılığını aynı kesitlerde karşılaştırır.
Gecikmiş
Retiküler veya mozaik enhans paterni gözlenir; venöz kollaterallerin ve damar obstrüksiyonunun detaylı değerlendirilmesi için çok fazlı inceleme gereklidir. Hepatik ven trombozunun görüntüleme değerlendirmesinde hepatik ven ve/veya VKI'deki oklüzyon ya da kompresyon ile venöz kollateraller aranır.

Önerilen pencereler: Batın (G 400 / M 50), Karaciğer (G 150 / M 30), Anjiyo (G 600 / M 150).

BT bulguları

  • Hepatik ven lümen kaybı — bir veya daha fazla hepatik venin seçilememesi, daralması ya da kısa bir kesimde sonlanması doğrudan çıkış tıkanıklığını düşündürür.
  • Venöz dolum defekti — hepatik ven veya VKI'deki oklüzyon ya da kompresyon, Budd–Chiari sendromunun doğrudan görüntüleme bulgularındandır.
  • Hepatik VKI lezyonu — suprahepatik VKI'de web, darlık, trombüs veya tümör uzanımı karaciğer çıkışını engelleyebilir.
  • Mozaik perfüzyon — portal fazda konjesyone ve görece korunmuş parankim alanları retiküler/yamalı kontrastlanma farkı oluşturur.
  • Kaudat lob hipertrofisi — VKI'ye doğrudan drene olan segment I büyür; kronik çıkış tıkanıklığında hacim yeniden dağılımının belirgin parçasıdır.
  • Veno-venöz kollateraller — hepatik venler arasında veya hepatik ven ile kaudat ven arasında kıvrımlı, kontrastlanan kanallar görülebilir.
  • Portal hipertansiyon bulguları — asit, splenomegali, genişlemiş portal ven ve gastroözofageal ya da karın duvarı kollateralleri eşlik edebilir.

Ölçütler ve sınıflamalar

Etiyolojik sınıflama
Primer Budd–Chiari, hepatik venöz sistem içinde gelişen obstrüksiyondur ve çoğunlukla trombozla ilişkilidir; sekonder Budd–Chiari damar dışından bası veya komşu tümörün damar içine uzanmasıyla gelişir.

Normalde

BT'de karaciğer damar anatomisini ve parankimal kontrastlanma paternlerini birlikte değerlendirin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
