Özet

İliokaval tromboz, alt ekstremite venöz dönüşünü ortak iliak venler ve vena cava inferior düzeyinde kesintiye uğratabilen proksimal derin ven trombozudur. BT ve BT venografi, trombüs yükünün yayılımını göstermeye ve eşlik eden anatomik nedenleri değerlendirmeye yardımcı olabilir. Geniş lomber, azygos ve paravertebral kollateraller kronik venöz obstrüksiyonu düşündürebilir; bu bulgular akut trombozla birlikte de görülebilir. Trombüsün renal venlere uzanımı ve iliokaval yayılımı, hastalığın kapsamını belirlemek için görüntülemede değerlendirilmelidir.

Gecikmiş tanısal
BT ve BT venografide iliak venler ile vena cava inferior boyunca trombüsün yayılımı değerlendirilir; eşlik eden kollateral venöz dolaşım da görülebilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40), Anjiyo (G 600 / M 150).

  • Ortak iliak ven ve vena cava inferior lümeninde devamlı izlenen kontrastla çevrili trombüs
  • Tromboze bir vende BT'de lümen içi dolum defekti görülebilir.
  • Trombüsün renal ven ostiumlarına veya ana femoral venlere doğru uzanım göstermesi
  • Kronik iliokaval obstrüksiyonda lomber, azygos ve paravertebral kollateral venler belirginleşebilir.
  • Presakral, internal iliak, lomber ve abdominal duvar venlerinde belirginleşmiş kollateral ağ
  • İliak venöz çıkış yolunu daraltan pelvik kitle veya komşu lenf nodu paketi

Normalde

BT, vena cava inferiorun ve renal venlerin anatomisini değerlendirmeye ve trombüsün yayılımını belirlemeye yardımcı olabilir. Kronik venöz obstrüksiyonda lomber, azygos ve paravertebral kollateral venler belirginleşebilir. BT, trombüsün IVC veya konjenital anomalilere uzanımı hakkında bilgi sağlar; kavagrafi ise iliokaval trombozun kriter standart tanı yöntemidir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

IVC'deki yalancı lezyonlar trombüsü taklit edebilir; opasite düzensizliği trombüs olarak yorumlanmadan önce bu olasılık da değerlendirilmelidir.
Bland tromboz ile tümör trombüsü ayırımı önemlidir; tümör trombüsü primer maligniteyle bağlantılıdır ve radyolojik karakterizasyonu çok parametrik görüntüleme araçlarıyla yapılabilir.
İzole iliak ven basısında ven dıştan komprese olabilir; basının derecesi venografi ve intravasküler ultrasonografiyle en iyi belirlenir.
Kronik posttrombotik daralmada, ven içinde organze olmuş trombus ve belirgin venöz kollateraller izlenir; bu bulgular akut genişlemiş ven görünümünden ayırt edilebilir.
