Özet

Multipl skleroz ve diğer demiyelinizan hastalıklar, beyaz cevher yollarını etkileyen inflamatuvar süreçlerdir; tanısal örüntü çoğunlukla MRI ile ortaya konur. Kontrastsız BT’de bazı plaklar komşu beyaz cevhere göre silik hipodens alanlar olarak seçilebilir, ancak çok sayıda aktif veya eski lezyon görünmeyebilir. Kontrastsız BT, demiyelinizan süreçlerin değerlendirilmesinde duyarsızdır; tanısal örüntü ve ayırıcı tanı için MRI esas alınır. Normal BT, demiyelinizasyonu dışlamaz; özgül olmayan beyaz cevher hipodensitesine bakarak da hastalık tanısı konmaz.

Okuma sırası

  1. Periventriküler beyaz cevher. Lateral ventrikül gövdesi düzeyinde beyaz cevheri tarayın; demiyelinizan bir lezyon kontrastsız BT’de hipodens görünebilir.
  2. Karşı hemisfer. Şüpheli alanı BT’de hipodens lezyon olarak tanımlayın; bu görünüm tek başına demiyelinizan hastalık tanısı koydurmaz.
  3. Gri-beyaz bileşke. Centrum semiovale düzeyinde beyaz cevheri inceleyin; demiyelinizan hastalığın tanısal değerlendirmesinde beyin ve omurilik MRG’si kullanılır.
  4. Lateral ventrikül ve orta hat. Ventrikülün basılıp basılmadığını ve orta hattın yerinde olup olmadığını inceleyin; kitle etkisi varsa tipik plak varsayımı yerine tumefaktif lezyon ve taklitçileri araştırın.

Faz ve pencere

Kontrastsız
BT’de görünür bir tumefaktif demiyelinizan lezyon kontrastsız incelemede hipodens olabilir; bu görünüm tek başına demiyelinizasyona özgü değildir. Kontrastsız BT, demiyelinizan lezyonların yaygınlığını ve aktivitesini değerlendirmede duyarsızdır; bu amaçla MRI tercih edilir.

Önerilen pencereler: Beyin (G 80 / M 40).

BT bulguları

  • Periventriküler beyaz cevher lezyonları MS’in MRI değerlendirmesinde önemlidir; tanıda klinik ve/veya MRI ile uzayda ve zamanda yayılım ölçütleri kullanılır.
  • Subkortikal odaklar — Kortekse komşu beyaz cevherde bir veya birden fazla hafif hipodens alan görülebilir.
  • Konfluent beyaz cevher değişikliği, lökodistrofi ve küçük damar iskemi kaynaklı lökoaraioz ile görüntüleme olarak örtüşebilir.
  • Korpus kallozum komşuluğu — BT’de kallozal plaklar çoğunlukla yeterince seçilmez; MRI sagittal FLAIR daha duyarlıdır.
  • Kitle etkisinin eşlik etmesi — küçük klasik plaklar belirgin orta hat kayması oluşturmaz; hacim etkisi varsa tumefaktif lezyon veya başka kitle taklitçisi araştırılır.
  • Negatif inceleme — normal kontrastsız BT, demiyelinizan hastalığı dışlamaz ve MRI incelemesini gereksiz kılmaz.

Normalde

Kontrastsız kafa BT’sinde gri ve beyaz cevher arasındaki atenüasyon farkını değerlendirirken, kalvaryuma yakın bölgelerde görüntü homojenliğinin etkilenebileceğini göz önünde bulundurun. Kontrastsız BT’de hipodens bir lezyon görüldüğünde, bunun demiyelinizasyon için özgül olmadığını ve tümör taklitçilerinin de ayırıcı tanıda bulunduğunu göz önünde bulundurun.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

periventriküler ve derin beyaz cevherde daha yaygın/konfluent hipodensite yapabilir; yaş ve vasküler risk profili bağlam sağlar.
Vazojenik ödem
kitle çevresinde beyaz cevher ağırlıklı hipodensite, sulkus ve ventrikül basısıyla birlikte olabilir.
Akut iskemik değişiklik
demiyelinizan hastalıklar inme taklitçileri arasında yer alır; ayrımda klinik değerlendirme ve MRI dahil nörogörüntüleme önem taşır.
Toksik-metabolik lökoensefalopati
iki taraflı yaygın beyaz cevher dağılımı ve maruziyet/metabolik öykü ayırıcıdır.
Enfeksiyöz ensefalit
temporal lob, insula veya limbik gri cevher ağırlıklı değişiklikler klinik ve dağılımla değerlendirilir.

Tuzaklar

  • Kontrastsız BT’nin normal oluşunu MS dışlama testi gibi kullanma; plakların önemli bölümü bu yöntemde görünmez.
  • Her periventriküler hipodensiteyi demiyelinizasyon sayma; yaşa bağlı küçük damar değişikliği ve ödem aynı bölgede düşük atenüasyon yapabilir.
  • Belirgin kitle etkisini sıradan plak olarak raporlama; geniş lezyonda tümör, enfeksiyon ve tumefaktif demiyelinizasyon MRI ile ayrıştırılmalıdır.
