Özet

Willis poligonu ve vertebrobaziler sistemdeki gelişimsel varyantlar, bir arter segmentinin ince ya da yok olmasından fetal kökenli dallanmaya ve fenestrasyona kadar değişen anatomik düzenlerdir. Akut inme BT anjiyografisinde bu yapılar oklüzyon, diseksiyon veya anevrizma sanılabileceği gibi, gerçek bir tıkanmada kollateral dolaşımın nasıl işleyeceğini de belirler. Kontrastsız BT damar lümenini güvenilir biçimde haritalamaz; damar ağacını izlemek için tanısal inceleme BTA'dır. Varyantı tanımadan raporlamak yanlış damar alanı atamasına, iki taraflı etkilenebilecek dokunun gözden kaçmasına veya girişim planında hataya yol açabilir.

Faz ve pencere

BTA tanısal
Kontrastlı BT anjiyografisinde A1 ve P1 segmentlerinin çapları, posterior kommunikan arter ile PCA arasındaki bağlantı derecesi ve arterlerdeki fenestrasyonlar ince MPR/MIP görüntülerinde karşılaştırılarak değerlendirilir.

Önerilen pencereler: Anjiyo (G 600 / M 150), Beyin (G 80 / M 40).

BT bulguları

  • A1 hipoplazisi veya aplazisi — bir ACA A1 segmenti belirgin ince ya da seçilemez; karşı A1, ACom üzerinden iki A2'yi besleyebilir.
  • Fetal PCA — PCA'nın PCom üzerinden internal karotisten çıkması ve ipsilateral P1'in küçük ya da izlenmez olması posterior serebral alanın karotis bağımlılığını gösterir.
  • Azygos ACA — iki A2 yerine orta hatta tek distal ACA gövdesi izlenir; bu gövdenin tıkanması bilateral medial frontal alanı etkileyebilir.
  • Bihemisferik ACA — tek taraflı dominant A2 karşı hemisfer medial yüzüne dallar gönderir; distal dallanma alanı karşı taraftan da izlenmelidir.
  • Gelişimsel tam kaynama bozukluğuna bağlı olarak ana arterde pürüzsüz duvarlı iki paralel lümen izlenir; bu yapı daralma veya anevrizma ile karışabilir.
  • Vertebral arterin PICA'da sonlanması — vertebral arter baziler birleşkeye ulaşmadan PICA dalını verir ve ince distal segmentle sonlanır.
  • Vertebral arter (özellikle V4 segmenti) veya baziler arter düzeyinde fenestrasyon — damarın kısa bir kesimde paralel iki kanala ayrılıp yeniden birleşmesi izlenir.

Normalde

Tipik CTA'da sağ ve sol A1 segmentleri ACom'a, iki A2 dalı ön dolaşıma; baziler arter ise iki P1 segmenti üzerinden PCA'lara devam eder. Kontrastlı BT anjiyografisi, A1 ve P1 segmentlerinde hipoplazi/aplazi, PCom varlığı ve arteriyel fenestrasyonlar dahil olmak üzere Willis poligonu varyantlarını sınıflandırmak için kullanılır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Gelişimsel varyantlar, ani sonlanma veya distal dolum kaybı olmaksızın kontinüe bir lümen gösterirken; oklüzyonda damar yolunun ani kesilmesi ve akımın durması beklenir.
İntrakraniyal anevrizmalar gelişimsel arter varyantlarıyla birlikte bulunabilir veya fenestrasyon bölgesinde ortaya çıkabilir; bu durum tanısal karmaşıklığa yol açar.
İntrakraniyal aterosklerotik stenoz
damar duvarında düzensiz daralma ve plak eşlik edebilir; gelişimsel ince segment genellikle düzgün konturludur ve anatomik dallanmayla uyumludur.
Diseksiyonda intraluminal akım katmanlanması, intramural hematom veya gerçek lümen daralması izlenir; gelişimsel varyantlarda bu akut duvar/yol değişiklikleri bulunmaz.

Tuzaklar

  • Küçük P1'i tek başına hipoplazi saymayın; fetal PCA yorumunda PCom'dan PCA'ya devamlılığı ve karşı tarafı birlikte izleyin.
  • A1 segmentinin görüntülenmemesi veya çok ince izlenmesi gerçek bir anatomik varyant olabileceği için karşı tarafla birlikte incelenmeli ve multiplanar görüntülerde takip edilmelidir.
  • Kompleks arter varyantların doğru tanımlanması için üç boyutlu rekonstrüksiyon veya rotasyonel anjiyografi kullanımı gereklidir; tek boyutlu projeksiyonlar veya basit hacim renderleri detayları gizleyebilir.
