Beyin sapı gliomları anatomik yerleşim ve klinik davranışlarına göre farklı örüntüler gösteren heterojen bir gruptur; diffüz intrinsik pontin gliom çocuklarda görülür ve pons merkezlidir. DIPG tanısında tercih edilen görüntüleme yöntemi MR’dır; bazı durumlarda BT de kullanılabilir. MR, tümörün komşu bölgelere infiltratif yayılımını değerlendirmede tercih edilen yöntemdir; gadolinyumlu görüntüler tanıyı destekleyebilir veya diğer lezyonları dışlamaya yardımcı olabilir. Beyin sapı kitle etkisinin atlanması BOS tıkanıklığı veya hızlı nörolojik kötüleşme riskinin gözden kaçmasına neden olabilir.
Okuma sırası
- Pontin parankim ve ventral kontur. Aksiyel kesitte pons merkezli intrinsik bir genişleme olup olmadığını değerlendirin; diffüz pontin gliomda tümör pons içinde merkezlenir.
- Prepontin sistern. Dördüncü ventrikül çıkışının tıkanması BOS çıkışını engelleyerek hidrosefaliye yol açabilir.
- Dördüncü ventrikül. Ponsun arkasındaki dördüncü ventrikülün şekil ve boyutuna bakın; ponsun içinden genişleyen tümörler ventrikülü basarak yassılaştırabilir veya deform edebilir.
- Akuadukt, lateral ventriküller ve periventriküler beyaz cevher. Son olarak obstrüktif hidrosefali bulgularını değerlendirin; bu durum tanı anında hastaların %10’undan azında görülür.

- Kontrastsız tanısal
- BT’de diffüz pontin gliom hipodens görünebilir. MR’da lezyon genellikle T1’de hipointens, T2/FLAIR’de hiperintens ve infiltratif sınırlıdır.
Önerilen pencereler: Beyin (G 80 / M 40), Yumuşak doku (G 400 / M 40).
BT bulguları
- Pons merkezli genişleme — diffüz pontin gliomda lezyon pons içinde yerleşir ve ponsun aksiyel çapının yarısından fazlasını tutabilir.
- Klasik MR örüntüsünde pons ekspansedir; lezyon ponsun aksiyel çapının yarısından fazlasını tutabilir ve baziler arteri sarabilir. DIPG tanısı klinik sunum ve nörogörüntüleme bulgularına dayanır.
- T1 hipointens/T2 hiperintens sinyal — diffüz pontin gliomda T1 ağırlıklı görüntülerde hipointens, T2 ağırlıklı görüntülerde ise belirgin hiperintens sinyal izlenir.
- Belirsiz kenarlar ve kistik/eksofitik bileşenin yokluğu — diffüz pontin gliomda tümör kenarları belirsizdir; kistik alanlar veya dışa doğru çıkıntılar (eksofitik büyüme) tipik değildir.
- Dördüncü ventrikül çıkışının tıkanması obstrüktif hidrosefaliye yol açabilir.
- Diffüz pontin gliomda BT’de hipodens lezyon görülebilir; tanıda tercih edilen görüntüleme yöntemi MR’dır.
Ölçütler ve sınıflamalar
- WHO CNS5’te “Diffüz orta hat gliomu, H3 K27-altered” olarak adlandırılır ve pediatrik-tip yüksek dereceli difüz gliom kategorisine alınır.
- WHO CNS5’te Diffüz orta hat gliomu, H3 K27-altered, ayrı bir histomoleküler tümör tipidir. DIPG, pons merkezli klinik-radyolojik örüntüyü ifade eder; H3 K27-altered tanısı için görüntülemenin yanında moleküler doğrulama gerekir.
Normalde
Aksiyel kesitlerde ponsun merkezî genişlemesini ve baziler arterin tümör tarafından sarılmasını değerlendirin; tümörün komşu bölgelere yayılımını da gözden geçirin. Pons içindeki tümör çekirdeğinin merkezlenmesini ve baziler arteri sarıp sarmadığını değerlendirin; infiltratif yayılım komşu bölgelere uzanabilir.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
