Özet

Bilinmeyen primerli metastatik hastalıkta görüntülemenin amacı, metastaz dağılımını tanımlamak, olası primer odağı aramak ve doku tanısı için uygun hedefi göstermektir. BT, CUP değerlendirmesinde başlangıç incelemesi ve evrelemenin temel görüntüleme yöntemidir. CUP, kapsamlı tanısal değerlendirmeye rağmen primer odağı belirlenemeyen metastatik hastalıkla tanımlanır. BT, primer tümörü saptama ve evrelemede temel yöntemdir; ancak bazı primer tümörler güncel tanısal incelemelere rağmen saptanamayabilir. Primer odağı ararken görüntüleme, lezyonları karakterize etmeye ve biyopsiye rehberlik etmeye yardımcı olabilir.

BT bulguları

  • BT'de saptanan bir kitle olası primer odağı düşündürebilir; yumuşak doku karmaşıklığı nedeniyle metastazlar primer tümör olarak yanlış sınıflandırılabilir.
  • Metastatik dağılımın görüntülenmesi, hastalığın yaygınlığı ve niteliği hakkında kapsamlı bilgi sağlar.
  • Servikal lenf nodu metastazında primer odağın araştırılması için tanısal değerlendirme BT veya MRG'yi içerebilir.
  • Peritoneal yayılım, ileri gastrointestinal veya jinekolojik kanserlerde görülebilir ve bu olasılıkların değerlendirilmesini gündeme getirir.
  • Biyopsi hedefinin seçimi örnekleme hatalarını etkileyebilir; tümör heterojenliği dikkate alınmalıdır.

Şüpheli odağı normal anatomik konturla karşılaştırın; metastazın en solid ve güvenli erişilebilir kısmını, damar ve barsak komşuluğundan ayrı olarak tarif edin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Granülomatöz hastalık
