Özet

Ektopik böbrek, normal lomber renal yatağın dışında yerleşir; füzyon anomalilerinde iki böbrek parankimi ya da alt kutupları birbirine bağlanabilir. Kontrastlı BT rekonstrüksiyonları gerçek renal dokuyu, normal yatağın boşluğunu, rotasyon bozukluğunu ve eşlik eden taş, hidronefroz veya travma bulgularını ortaya koyar; arteriyel görüntüleme gerektiğinde değişken damar kökenlerini haritalayabilir. Çocukta radyasyon nedeniyle tanı ve takipte ultrason veya MR çoğu durumda önce değerlendirilir; BT özellikle karmaşık anatomi ve akut komplikasyon sorularında katkı sağlar. Böbreği agenezi sanmak, ektopik üreteri veya anormal damarları fark etmemek cerrahi ve girişimsel işlemlerde önemli anatomik hatalara yol açabilir.

Faz ve pencere

Portal tanısal
Kontrastlanan renal parankimin normal böbrek yatağı dışındaki pelvis, iliak bölge veya karşı taraftaki yerleşimi; at nalı böbrekte alt kutuplar arasındaki parankimal köprü, anormal uzun eksen/rotasyon ve varsa hidronefroz, taş ya da travma bulguları değerlendirilir.
Arteriyel
Damar haritalaması istenmişse ektopik renal arterlerin aorta, distal aorta veya iliak arterlerden kökeni ve füzyone böbreğe giden değişken dallar izlenir; rutin pozisyon tanısı için bu faz zorunlu değildir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40), Anjiyo (G 600 / M 150).

BT bulguları

  • Boş renal yatak — beklenen böbrek lojunda renal parankim görülmez; agenezi demeden önce alt abdomen ve pelvik yerleşimler taranır.
  • Pelvik veya iliak ektopik böbrek — kontrastlanan renal doku normal lomber seviyenin altında, çoğu kez malrotasyonlu olarak seçilir.
  • Karşıya geçmiş böbrek — çapraz ektopide iki renal ünite aynı tarafta yerleşebilir; üreterlerin mesaneye uzanan seyri ve parankimal füzyon durumu ayrıca değerlendirilir.
  • At nalı biçimli alt kutup köprüsü — iki böbreğin alt kutupları orta hattı geçen parankimal veya fibröz istmusla birleşir.
  • Anormal rotasyon — renal pelvis ve hilus öne veya yana bakabilir; uzun eksen ve kalikslerin yönü olağan konumdan farklıdır.
  • Değişken renal arterler — damarlar distal aorta, iliak arterler veya çoklu dallardan çıkabilir; parankimin kontrastlanma alanıyla ilişkileri değerlendirilir.
  • Eşlik eden boşaltım sistemi bulguları veya akut sorun — üreteropelvik bileşke genişlemesi, taş, enfeksiyon ya da travmatik parankim hasarı ektopik veya füzyonlu anatomide ayrıca araştırılır.

Normalde

Normal böbrekler sağ ve sol renal yataklarda, omurganın iki yanında benzer seviyede ve hilusları mediale dönük bulunur; iki ayrı organ arasında orta hattı geçen parankimal köprü olmaz. Aynı koronal BT'de boş renal yatağı, pelvik/karşı taraftaki renal parankimi ve varsa alt kutuplar arasındaki istmusu takip edin; karşılaştırmada yalnız konumu değil, hilus yönünü ve damar girişlerini de kullanın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
