Özet

Patolojik öncü nokta, bağırsak duvarından lümene uzanarak invajinasyonu başlatabilen yapısal bir odaktır; çocukta çoğu invajinasyon idiopatik olsa da odak varlığı özellikle atipik veya tekrarlayan tabloda önem taşır. Ultrason çoğu çocukta tanı ve öncü nokta araştırmasının ilk yöntemidir; seçilmiş olguda BT, invajinasyon merkezindeki kitleyi ve çevresindeki bağırsak-mezenter anatomisini gösterir. BT'de yalnız invajinasyonun varlığını değil, kitlenin duvarla bağlantısını ve ayrı bir odak olup olmadığını aramak gerekir. Öncü noktanın saptanması, invajinasyona yol açan altta yatan lezyonun tanınmasına yardımcı olur; invajinasyon bağırsak tıkanıklığıyla birlikte görülebilir.

Faz ve pencere

Portal tanısal
İntravenöz kontrastlı BT'de invajinasyon merkezinde duvarla devamlılığı bulunan yumuşak doku odağı, polipoid kitle veya ektopik bağırsak uzantısı; çevresel duvar kalınlaşması, mezenterik lenf düğümleri ve proksimal tıkanma araştırılır.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • İnvajinasyon tepesinde olası öncü noktayı arayın; BT'de yağ dansitesinde intramural kitle veya terminal ileuma boyunla bağlı kör sonlanan divertiküler yapı görülebilir.
  • Meckel divertikülü — distal ileumdan çıkan kör sonlanan bağırsak benzeri kese, invajinasyonun tepesinde ya da çekilen segment boyunca izlenebilir.
  • Polipoid lezyon — lümene doğru uzanan sınırlı yumuşak doku kitlesi, invajinasyon ekseninde çekinti odağı oluşturur.
  • Duvar kaynaklı kitle — fokal veya asimetrik bağırsak duvar kalınlaşması lümene doğru çıkıntı yapar ve komşu segmentten ayrışır.
  • Tekrarlayan ileokolik invajinasyonlu bir olguda komşu mezenterde reaktif, yuvarlak lenf düğümleri izlenmiştir.
  • Proksimal ince bağırsak anslarında genişleme, invajinasyona eşlik eden mekanik tıkanıklığı destekler.

Normalde

Normal ince bağırsakta duvar kalınlığı ve kontrastlanması segment boyunca düzenlidir; lümene uzanan ayrı kitle, divertiküler kese veya fokal çekinti görülmez. İnvajinasyon merkezindeki mezenterik düğümleri duvarla devamlılığı olan odaktan ayır ve aynı segmentin karşı duvarıyla kalınlık-kontrast farkını kıyasla.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
