Özet

Çok düzlemli rekonstrüksiyon (MPR), hacimsel BT verisinden koronal, sagittal veya anatomiye göre seçilmiş oblik kesitler üretir. Tıp öğrencisi ve acil hekimi, örneğin böbrek, damar ya da bağırsak gibi aksiyelde eğik seyreden yapının devamlılığını başka bir düzlemde izleyebilir; ölçüm düzlemi ise hedef anatomiyi tam olarak yansıtacak şekilde kurulur. Rekonstrüksiyon yeni bir edinim değildir, mevcut voksel verisinin yeniden örneklenmesidir; ince kesit ve uygun uzaysal çözünürlük görüntü kalitesini belirler. Yapının uzun eksenine eğik düzlemde ya da kalın kaynak kesit üzerinde yapılan ölçümde, lezyon boyutu olduğundan büyük çıkabilir ve yapısal ilişkiler yanlış aktarılabilir.

Faz ve pencere

Kontrastsız tanısal
MPR için kontrast fazı gerekmez; aynı hacimsel BT verisi aksiyel kaynak kesitlerden koronal, sagittal veya oblik planda yeniden örneklenir. Bu eğitim örneğinde böbrekler ve renal pelvisin tek hacim içinde kesintisiz izlenmesi üzerinden düzlem seçimi gösterilir.

BT bulguları

  • Ortogonal düzlemler — aksiyel kaynağa dik koronal ve sagittal düzlemler, aynı anatomik hacimde karşılıklı kesit çizgileriyle eşleştirilir.
  • Oblik düzlem — seyrini birden çok düzlemde sürdüren yapılar boyunca kurulan MPR, tek düzlemde tam izlenemeyen anatomik sınırları tek görüntüde temsil eder; kalın kaynak kesitteki parsiyel hacim etkisini azaltır.
  • İzotropik örnekleme — voksellerin üç boyuttaki boyutları birbirine yaklaştıkça farklı düzlemde keskinlik daha iyi korunur; kalın kaynak kesitte dikey düzlem bulanıklaşabilir.

Normalde

Normal böbrek BT'sinde renal korteks ve medulla sınırları keskin, renal sinüs yağ planları korunmuş ve pelvis hilusa doğru toplanmış görünür. Sağ ve sol böbreği ayrı ayrı, aksiyel, koronal ve sagittal panellerde aynı voksel düzeyinde eşleyin; korteksin dış sınırını, renal sinüsün merkezini ve pelvisin hilustaki devamını kıyaslayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Kalın aksiyel kaynak kesitlerden ince sagittal görüntü üretmek yeni uzaysal ayrıntı sağlamaz; kortikal çizgiler ve küçük damarlar bulanık kalabilir.
  • Oblik reformatın düzlem yönünü göstermeden çap raporlamayın; renal uzun eksene paralel düzlemde lezyon olduğundan uzun görünebilir, dik düzlem ise kısa eksenini verir.
  • Çoklu düzlemli korelasyonda referans çizgilerinin aynı anatomik düzeyi kestiğinden emin olun; bağımsız yeniden örnekleme veya kalın kaynak kesitler nedeniyle bozulan uzunlamasına çözünürlük, anatomik devamlılık yanılgısına yol açabilir.
  • MPR'de daha belirgin görünen sınırı daha yüksek çözünürlük sanmayın; rekonstrüksiyon mevcut hacmi başka yönden örnekler, edinim kaynaklı bulanıklığı gidermez.
