Özet

Piramidal lob, tiroid bezinin üst kutbundan veya istmustan yukarı uzanan embriyolojik kalıntısıdır ve rutin boyun BT'sinde beklenmedik bir doku çıkıntısı gibi görülebilir. Önemi, tiroid dokusunun gerçek kraniyal sınırını ve cerrahi ya da nükleer tıp değerlendirmesinde geride kalabilecek dokuyu göstermesidir. BT'de en güvenilir ipucu, uzantının bezle aynı yoğunluk ve kontrastlanmayı göstermesi ve multiplanar görüntülerde tiroid dokusuna bağlanmasıdır. İsthmus'a veya loba bağlılığı kopuk (%10 civarında) görünebilir; bu uzantıda gizli malign nodüller saptanabilir.

Kontrastsız tanısal
Boyun BT'sinde tiroidin istmusundan veya loblarından kraniye uzanan, tiroid parankimiyle aynı densitede yumuşak doku uzantısı değerlendirilir; çoğunlukla sol loptan veya istmustan köken alır.

Önerilen pencereler: Yumuşak doku (G 400 / M 40).

BT bulguları

  • İstmusun veya bir lobun üst kutbunun ön yüzünden kraniyale uzanan dilsi ya da konik tiroid dokusu.
  • Uzantı, sagittal ve koronal rekonstrüksiyonlarda ana bezle kesintisiz doku köprüsü oluşturur.
  • Genellikle sol tiroid lobunun medial yüzünden veya istmusun sol yanından kraniye uzanır; skintigrafide ve BT'de sol kökenli görülme sıklığı daha yüksektir.
  • BT ölçümleriyle uzantının uzunluğu, çapı ve hacmi belirlenebilir; ana bezle olan kesintisiz doku bağı veya ayrılığını değerlendirmek tanıyı sağlar.
  • Yüksek kesitlerde uzantının üst sınırı genellikle tiroid kıkırdağı seviyesinde saptanır; bu boyutlandırmalar preoperatif planlama için kullanılabilir.
  • Uzantı içinde nodül varsa bezin geri kalanından farklı kontur veya iç yapı odağı oluşturabilir; bu bulgu varyantın kendisinden ayrı değerlendirilir.

Normalde

Normal tiroidde sağ ve sol lob, istmusla birleşir; hyoid seviyesine doğru ayrı bir parankim dili uzanmaz. Aynı aksiyel düzeyin üzerindeki kesitleri koronal veya sagittal düzlemde izleyip beklenen kraniyal bez sınırının devam edip etmediğine ve uzantı yoğunluğunun tiroidle eşleşmesine bak.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Santral kompartıman (Delphian) nodu, piramidal lob papiller karsinomuyla klinik ve radyolojik benzerlik gösterebildiğinden ayırıcı tanıda düşünülmelidir.

  • Hyoid kemiğe uzanan fibromüsküler levator glandulae thyroideae bandı, parankimal piramidal lobdan farklı bir anatomik yapıdır; hyoide uzanım tek başına tiroid parankim uzantısını göstermez.
  • Piramidal uzantının içinde nodül bulunması mümkündür; uzantının varyant olması içindeki fokal lezyonu otomatik olarak benign yapmaz.
