Özet

Önceden var olan tiroid nodülü veya kisti içine kanama, boyunda hızlı büyüyen şişlik ve bası yakınmasıyla acile başvurabilir. BT, kan ürünleri içerebilen kompleks kistik-solid lezyonu gösterir ve özellikle trakea lümeni, mediastene uzanım, damar basısı ile olası aktif ekstravazasyonu ortaya koyar. Kanama görünümü iyi huylu nodüler zeminde oluşabilse de BT tek başına eşlik eden tümörü dışlayamaz. Hava yolu daralmasının veya aktif kanamanın fark edilmemesi hızla ilerleyen solunum sıkıntısının ve boyun hematomunun ciddiyetinin eksik değerlendirilmesine yol açabilir.

Faz ve pencere

Kontrastsız tanısal
Kontrastsız boyun BT'sinde kist veya nodül içinde kan ürünü ile uyumlu hiperdens içerik ve bez hacminde asimetrik artış izlenir; lezyonun yoğunluğu kanamanın evresine bağlı olarak değişiklik gösterebilir.
BTA tanısal
Aktif kanama şüphesinde arteriyel BT anjiyografi serisinde nodül ya da çevre hematom içinde damar havuzuna benzer, damar dışına taşan kontrast odağı aranır; incelemenin asıl rolü aktif ekstravazasyonu ve olası besleyici damarı göstermektir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Anjiyo (G 600 / M 150).

BT bulguları

  • Tiroid lobunda önceden var olan kistik veya kompleks nodül içine yeni hiperdens materyal birikmesi.
  • Bez hacminde tek taraflı hızlı genişleme ve tiroid kapsülünün dışa bombeleşmesi.
  • Farklı yoğunlukta kan ürünleri veya pıhtılaşma nedeniyle heterojen içerik ve lokal hacim artışı görülebilir.
  • Trakea lümeninde daralma, karşı tarafa itilme veya larenks-alt farinks düzeyine uzanan bası etkisi.
  • Retrosternal uzanım ve üst mediastene geçen kanama, boyun dışından görülen şişlikten daha geniş hastalık alanı oluşturabilir.
  • BT anjiyografide nodül içindeki fokal kontrast kaçağının arteriyel havuzla benzer yoğunlukta belirmesi aktif kanamayı destekler.
  • Juguler ven veya karotis arterde yer değiştirme ve bası görülebilir; eşlik eden tromboz ya da damar duvarı yaralanması ayrıca aranır.

Normalde

Kaynaklar hastada difüz büyüklükte kompleks kistik-solider nodüllerin trakeayı sıkıştırıp sağa ittiğini belirtmektedir; normal bezde bu asimetrik genişleme ve trakeal bası görülmez. Lezyonun boyutları ve retrosternal uzanımı, trakeada sağa kaymaya neden olan kütle etkisi ile izlenir; aşağıya doğru takip gerekebilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Enfekte tiroid kisti veya apse
çevresel inflamasyon, kalın kontrastlanan duvar ve çevre yağ planlarında kirlenme eşlik edebilir; klinik enfeksiyon bulguları ayrımı destekler.
Hemorajik dejenerasyon gösteren nodüller heterojen veya kistik yapıda olabilir; ancak aktif kanama şüphesinde hiperdens içerik ve akut hacim artışı ayırt edici bulgulardır.
Tiroid malignitesi içine kanama
düzensiz solid komponent, ekstratiroidal yayılım veya şüpheli nodlar eşlik edebilir; hemoraji benignliği kanıtlamaz.
Ekstratiroidal boyun hematomu
retropharyngeal alanda yerleşip trakeayı öne itebilir.
Aktif tiroid arteri kanaması veya psödoanevrizma
kontrastlı seride damar lümeniyle bağlantılı ekstravazasyon ya da fokal damar kesesi izlenebilir.

Tuzaklar

  • Kontrastlı tek seride hiperdens odak kan, kalsifikasyon veya damar içi kontrast olabilir; varsa kontrastsız görüntüyle eşleştir ve seri kesitlerde damar bağlantısını izle.
  • Kontrastsız veya kontrastlı BT'de hiperdens odaklar kan ürünü, kalsifikasyon veya aktif ekstravazasyonu temsil edebilir ve bu durum altta yatan nodülün iyi huylu veya malign yapısını kesin olarak ayırt etmeyi zorlaştırabilir.
  • Trakeanın hafif yer değiştirmesi lümenin güvenli olduğu anlamına gelmez; daralma ve larenks seviyesine uzanım ardışık kesitlerde ayrıca incelenmelidir.
