Özet

Optik sinir kılıfı menenjiyomu çoğunlukla erişkinde, optik sinir çevresindeki araknoid kılıftan gelişen ve yavaş büyüyerek görmeyi tehdit eden bir tümördür. BT, özellikle kılıf kalsifikasyonunu ve optik kanal ile komşu kemikteki değişiklikleri gösterir; yumuşak doku yayılımını değerlendirmede kontrastlı, yağ baskılı MR daha duyarlıdır. Sinir merkezde korunurken çevresinde kılıf dokusunun görülmesi, sinirin kendi içinden büyüyen optik gliomadan ayrımın temelidir. Kanal içi uzanım veya apeks tutulumu gözden kaçarsa hastalığın intrakraniyal devamlılığı eksik değerlendirilebilir.

Faz ve pencere

Kontrastsız tanısal
İnce kesit orbital BT'de optik siniri çevreleyen kılıfta noktasal, çizgisel ya da tram-track biçimli kalsifikasyon; optik kanal korteksinde genişleme veya hiperostoz araştırılır. Kılıf yumuşak dokusu kontrastsız BT'de sınırlı seçilebilir; kalsifikasyon ve kemik değerlendirmesi bu fazın katkısıdır.
Kemik algoritması
Optik kanalın iki taraflı çap ve korteks görünümü, sfenoid kemikte hiperostoz ve kanal boyunca kemik remodeling değerlendirilir; kılıf tümörünün kendisini dışlamak için tek başına yeterli değildir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • Optik sinirin çevresinde ince kalsifiye kılıf, aksiyel kesitte iki paralel yoğun çizgi görünümü oluşturabilir.
  • Kılıf boyunca uzanan kalsifikasyon, sinirin merkezde görece korunmasıyla birlikteyse tram-track görünümü verir.
  • Tümör tübüler, globüler ya da fokal kılıf kalınlaşması biçiminde olabilir; BT kılıfın yumuşak doku sınırlarını her zaman göstermez.
  • Optik kanal içinde kitle veya kanal genişlemesi intrakanaliküler uzanımı düşündürür; normal kanal biçimi hastalığı dışlamaz.
  • Komşu sfenoid kemikte hiperostoz menenjiyom lehine destekleyici kemik bulgusudur.
  • BT'de kalsifikasyon ve tram-track görünümü tanıda belirleyici olabilir ancak tüm vakalarda gözlenmeyebilir.

Normalde

Optik sinir intraoküler, intraorbital, intrakanaliküler ve intrakraniyal olmak üzere dört segmentten oluşur ve toplam uzunluğu yaklaşık 50 mm'dir. Orbital lezyonların doğru yorumlanması standart terminoloji kullanımı gerektirir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Görüntülemede menenjiom tanısı mükemmel ayırım sağlamaz; biyopsi ile histolojik kompozisyon beklenenden farklı çıkabilir.
  • Küçük veya kalsifiye olmayan tümörde normal kontrastsız BT görünümü hastalığı dışlamaz; şüphe sürüyorsa MR ile sinir-kılıf ilişkisini göster.
  • Orbital lezyonların yorumlanması standart terminoloji gerektirir.
