Orbital nörofibrom / pleksiform nörofibrom

Özet

Orbital nörofibrom sinir kılıfı kökenli benign tümördür; pleksiform biçim özellikle NF1 ile güçlü ilişkilidir ve orbitaya yayılarak göz hareketlerini, görmeyi ve yüz görünümünü etkileyebilir. BT, kıvrımlı infiltratif yumuşak doku büyümesini, kemik foramen genişlemesini, sfenoid kanat displazisini ve orbital duvar remodelingini gösterir; sinir ve yumuşak doku uzanımını belirlemede MR daha duyarlıdır. Pleksiform görünüm ve kemik değişiklikleri hastalığın kapsamını ve eşlik eden NF1 bulgularını araştırmayı gerektirir. Hızlı büyüyen veya ağrılı kitlede malign dönüşüm olasılığını ve optik yapı basısını gözden kaçırmamak önemlidir.

Faz ve pencere

Kontrastsız

Ekstraoküler kaslara göre izodens/hipodens olabilen fusiform veya serpijinöz yumuşak doku, üst orbital fissür veya infraorbital kanal/foramen genişlemesi, sfenoid kanat displazisi ve orbital duvar remodelingine bakılır.

Kontrastlı tanısal

Kontrast sonrası nörofibromda değişken, pleksiform infiltratif dokuda sıklıkla heterojen boyanma; sinir hattı boyunca yayılım ve orbital yumuşak dokularla ilişkisi incelenir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

• Lokalize nörofibromda sinir doğrultusunda uzanan düzgün sınırlı oval veya fusiform kitle
• Pleksiform nörofibromda kapsülsüz, serpijinöz ve dallanan yumuşak doku büyümesi
• Orbital yağdan göz kapağına ve periorbital yumuşak dokuya uzanan infiltratif komponent
• Kontrast sonrası değişken veya heterojen boyanma; tek tip boyanma beklenmemesi
• Yerleştiği sinir hattına göre üst orbital fissür ya da infraorbital kanal/foramende genişleme veya remodeling
• Büyük sfenoid kanat displazisi, orbital duvar remodelingi ve buna bağlı glob konum değişikliği
• Proptoz, glob deplasmanı ve optik sinir/görme yoluna kitle etkisi

Normalde

Normal orbitte orbital yağ planları temiz ve sfenoid kanat konturu sağlamdır. Aynı seviyede karşı taraf sinir çevresi dokuyu, üst orbital fissür ve infraorbital kanal/foramen kemik sınırlarını, büyük sfenoid kanadı ve globun konumunu karşılaştır; pleksiform lezyonda yalnız tek bir kitle değil, dallanarak yayılan yumuşak doku hattını izle.

Ayırıcı tanı

Orbital schwannom

daha sık düzgün sınırlı, kapsüllü ve fokal oval/fuziform kitle görünümündedir.

Optik sinir gliomu

optik sinir boyunca tübüler genişleme yapar; eşlik eden NF1 bulguları ayırıcı olmayabilir.

Optik sinir kılıfı menenjiyomu

siniri çevreleyen kılıf kaynaklı tübüler lezyon ve kanal uzanımı gösterir.

Lenfatik/venöz malformasyon

çok odacıklı damar boşlukları, sıvı seviyeleri veya Valsalva ile değişen venöz komponentle ayrılır.

Malign periferik sinir kılıfı tümöründe yeni ağrı, hızlı büyüme, düzensiz sınırlar veya kemik infiltrasyonu malign dönüşüm kuşkusu uyandırır.

Tuzaklar

• Pleksiform nörofibromu tek bir kapsüllü orbital kitle gibi ölçüp sınırlandırma; dallanan yumuşak doku kapak ve yüz dokularına taşabilir.
• NF1 hastasında sfenoid kanat displazisi, tümörün doğrudan kemik invazyonu anlamına gelmez; kemik defekti ve remodeling biçimini ayrı tarif et.
• Sinir hattındaki her kitleye nörofibrom deme; düzgün sınırlı schwannom ile pleksiform, infiltratif büyüme örüntüsünü ayır.
• Ağrı veya hızlı büyüme varsa önceki görüntülerle karşılaştır; rutin BT'deki boyut tek başına malign dönüşümü dışlamaz.
