Özet
Alerjik fungal rinosinüzit, çoğunlukla atopik ve bağışıklığı sağlam kişilerde fungal antijenlere karşı gelişen, sinüslerde eozinofilik müsin ve nazal polipozisle seyreden inflamatuvar tablodur. Kontrastsız sinüs BT'si hiperdens müsin dağılımını, birden çok sinüsteki tıkanmayı ve basınçla oluşan genişleme/remodelingi haritalar. BT, klinik alerji ve cerrahi materyalin histopatolojisi olmadan tek başına tanıyı kesinleştirmez; görüntüdeki ekspansiyon invazyonla eş anlamlı değildir. Orbita veya kafa tabanına uzanımı kaçırmak anatomik etkilenmeyi eksik gösterirken, düzgün remodelingi agresif kemik invazyonu diye adlandırmak hastalığı yanlış sınıflandırabilir.
Faz ve pencere
- Kontrastsız tanısal
- Aksiyel ince kesit ve koronal rekonstrüksiyonlarda birden çok sinüste heterojen, hiperdens müsin; çevresel düşük yoğunluklu mukoza, ostiomeatal tıkanma ve düzgün ekspansil kemik remodelingi aranır. HU ölçümü tanıya destekleyici parametre olarak kullanılabilir ancak eşik değerler çalışmalarda değişkenlik gösterir.
Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).
BT bulguları
- Etmoid hücreler başta olmak üzere birden çok sinüste asimetrik, bazen tek taraflı olabilen opasifikasyon.
- Sinüs lümeninde heterojen hiperdens müsin ile havalanmış sinüs alanları arasındaki yoğunluk farkı belirgin olabilir.
- Sinüs lümenindeki heterojen hiperdens müsin ile komşu anatomik yapılar arasındaki kontrast dikkat çekicidir.
- Nazal kavitede polipoid yumuşak doku ve ostiomeatal komplekslerde tıkanma.
- Sinüs duvarlarında hacim artışı, incelme ve düzgün basınç remodelingi komşu anatomik konturları yer değiştirebilir.
- Lamina papyracea veya kafa tabanında düzgün incelme/erozyon görülebilir; bu tek başına histolojik invazyon göstermez.
- Orbital yağ planı, ekstraoküler kaslar ve kafa içi yumuşak dokular çoğunlukla korunur; gerçek infiltrasyon alternatif veya ek invaziv hastalık düşündürür.
Ölçütler ve sınıflamalar
- Bent–Kuhn tanı ölçütleri
- Bent–Kuhn'un beş klasik ana ölçütü: tip I aşırı duyarlılık, nazal polipozis, karakteristik BT görünümü, doku invazyonu olmaksızın eozinofilik müsin ve sinüs içeriğinde pozitif fungal boyama. Fungal kültür destekleyici/minör ölçüttür; BT tek başına tanı koydurmaz.
Normalde
Normal sinüsler havalıdır; duvarları ince, nazal pasajlar açıktır ve lamina papyracea ile kribriform plak kesintisizdir. Hastalıklı sinüsleri aynı taraftaki orbita ve kafa tabanı komşuluğuyla kıyaslayıp duvarın düzgün dışa remodelingi ile komşu yağ dokusuna uzanan infiltrasyonu ayır.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı
- Fungal top, sıklıkla tek taraflı sinüzit şeklinde görülür; en sık maksiller sinüsü tutar ve BT’de mikrokalsifikasyon paternleri gösterebilir.
- Kronik rinosinüzit ve polipozis
- drenaj yolu tıkanması yapar, ancak heterojen hiperdens müsin AFRS lehine daha ayırt edicidir.
- Mukosel
- bir sinüste ostium tıkanmasına bağlı homojen genişleme ve düzgün kemik incelmesi yapabilir.
- Akut invaziv fungal rinosinüzit
- periantral yağ, pterigopalatin fossa veya orbita infiltrasyonu ve nekrotik kontrastlanmayan mukoza gösterebilir.
- Sinonazal tümör
- solid kitle, düzensiz kemik yıkımı veya infiltratif komşuluk yapabilir; yalnız hiperdens sinüs içeriği tümör göstergesi değildir.
Tuzaklar
- Hiperdens içerik AFRS'ye özgü değildir; tek sinüs ve kalsifiye agregat fungal topu, diş materyali ise odontojenik katkıyı gündeme getirir.
- Sinüs ekspansiyonu ve düzgün duvar incelmesi basınç remodelinginden kaynaklanabilir; invazyon için orbital yağ, periantral alan, pterigopalatin fossa ve kontrastlanan mukozayı da incele.
- AFRS çoğu kez çoklu sinüs tutsa da tek taraflı/asimetrik olabilir; bilateralite yokluğu tek başına dışlatmaz.
- BT'deki hiperdens müsin tanı bileşenlerinden biridir; görüntüden fungal türü, alerjiyi veya histolojik invazyonu çıkarma.

Alerjik fungal rinosinüzit. Çoklu sinüs tutulumu, hiperdens müsin, polipozis ve düzgün remodelingin birleşimi alerjik fungal örüntüyü destekler. Fungal top genellikle tek sinüs odaklıdır; invaziv hastalıkta infiltrasyon/nekroz, mukoselde ise tek boşluğun genişlemesi ön plandadır.
AFRS olasılığında lamina papyracea incelmiş, ancak orbital yağ kesintisiz. Bu kemik bulgusu en iyi nasıl yorumlanır?
