Özet
Kronik granülomatöz invaziv fungal rinosinüzit, çoğunlukla bağışıklığı normal kişilerde bildirilen, yavaş gelişen ve granülom oluşturan invaziv fungal hastalık biçimidir; etken birçok seride Aspergillus flavus'tur. BT tek taraflı sinonazal kitleyi, kemik değişikliklerini ve özellikle orbitaya ya da kafa tabanına uzanımı göstererek biyopsi ve yayılım değerlendirmesini yönlendirir. Homojen veya heterojen kontrastlanan kitle görünümü tümörleri taklit edebilir; BT doku tanısının yerini tutmaz. Orbita veya intrakraniyal uzanımın atlanması görme ve nörolojik işlevi tehdit eden hastalığın kapsamını olduğundan küçük gösterebilir.

- İnce kesit kemik ve yumuşak doku rekonstrüksiyonlarında tek taraflı sinonazal kitle, sinüs opasifikasyonu, hiperdens fungal içerik ve düzgün remodelingle birlikte fokal erozyon/destrüksiyon aranır.
- Kontrastlı tanısal
- İntravenöz kontrast sonrası yumuşak doku serisi, solid granülomatöz kitlenin kontrastlanmasını ve orbitaya, pterigopalatin fossaya ya da kafa tabanına uzanımını gösterir; kesin tanı histopatoloji gerektirir, görüntüleme bulguları etkeni veya maligniteyi ayırt edemez.
Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).
BT bulguları
- Çoğunlukla kitle oluşturucu (mass-forming) sinonazal yumuşak doku lezyonu.
- Maksiller, etmoid veya frontal sinüs opasifikasyonu ve komşu nazal kaviteye devamlılık.
- Sinüs duvarlarında basınç remodelingi, skleroz veya yer yer kemik erozyonu/destrüksiyonu.
- Orbita tabanını ya da lamina papyraceayı aşan kitle ve orbital yağ içinde yumuşak doku.
- Ekstraoküler kas, orbital apeks veya optik kanal komşuluğuna uzanım ileri yayılımı gösterir.
- Kontrastlı incelemelerde invaziv fungal enfeksiyonlarda kontrast kaybı (LoCE) görülebilir; bu bulgu invazyonu işaret eder ve mikrobiyolojik doğrulama için erken cerrahi müdahaleyi gerektirir.
- Periantral yağ ve pterigopalatin fossa infiltrasyonu ekstrasinüs yayılımını düşündürür.
Normalde
Normal nazal kavite ve sinüslerde hava baskındır; mukozal çizgi ince, lamina papyracea ile orbital taban korteksi kesintisizdir. Tek taraflı veya pansinüzit opasifikasyonları değerlendirilir; sinüs duvarlarında basınca bağlı yeniden şekillenme ile invaziv hastalığa özgü fokal kemik erozyon/destrüksiyon ayrımı yapılır.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

- Lenfoma
- yumuşak doku kitlesi yapar; kemik yıkımı hacme göre sınırlı olabilir, tanı doku örneklemesine dayanır.
Tuzaklar
- Granülomatöz fungal kitleyi yalnız kontrastlanmasına göre malign tümörden ayırma; görünüm örtüşür ve histoloji gerekir.
- Kemik remodelingi yavaş büyümeyi düşündürür ama invazyonu dışlamaz; orbital yağ ve kasları ayrıca izle.
- Hiperdens sinüs içeriğini fungal etkenin özgül işareti sayma; proteinli sekresyon ve noninvaziv fungal içerik benzer olabilir.
